国内外日间手术发展现状与思考

Current Situation and Thoughts on the Development of Ambulatory Surgery at Home and Abroad

新医改要求现代医院管理要从粗放的规模扩张型向精细化的内涵质量效益型转变,积极探索并大力发展日间手术对医院转型发展有着积极的推动作用。从国家政策、运营管理模式、支付与补偿方式等方面,对国内外日间手术政策背景、发展现状及存在问题进行了分析,发现目前日间手术发展存在缺乏统一的准入标准、质量管理与绩效评价体系不健全、支付报销政策不完善等问题,并提出在政策引导与扶持、管理及准入标准、质量管控及评价体系以及医院-社区双向转诊通道建立等方面的改进建议,为规范发展日间手术、提升医疗服务效能与品质提供借鉴与参考。

The health care system reform requires transformation in modern hospital management from extensive scale expansion to refined connotation.Active exploration and development on ambulatory surgery plays a positive role in promoting the hospitals’transformation and development.This paper analyzed the policy background,the current situation and existing problems of ambulatory surgery in domestic and overseas from the aspects of national policy,operation management mode,payment and compensation,etc.It pointed out that there were some problems in the development of ambulatory surgery,such as the lack of unified access standard,the unsound quality management and performance evaluation system,and the imperfect payment and reimbursement policies.It also put forward suggestions on policy guidance and support,management and access standards,quality control and evaluation system,as well as the establishment of two-way referral channels between hospitals and communities,so as to provide reference for standardizing the development of day surgery and improving the efficiency and quality of medical service.
