彩色硅胶罐联合运动疗法治疗卒中后偏侧忽略临床研究

Clinical Study of Colored Silica Gel Can Combined with Exercise Therapy for Unilateral Neglect After Stroke

目的观察彩色硅胶罐联合运动疗法治疗卒中后偏侧忽略的临床疗效。方法选取卒中后偏侧忽略患者60例,采用随机数字表法将患者分为两组,每组30例,治疗组采用彩色硅胶罐联合运动疗法治疗,对照组采用单独运动疗法治疗,统计分析两组患者的单侧空间忽略各项参数、凯瑟琳-伯哥量表(CBS)评分、简易智能精神状态检查量表(MMSE)评分、Fugl-Meyer(FMA)运动功能评定量表评分、改良Barthel指数(MBI)评分、满意度。结果两组治疗前平面图形临摹试验、画钟试验、线段划销试验、直线二等分试验评分比较差异无统计学意义(P>0.05);两组治疗后平面图形临摹试验、画钟试验、线段划销试验、直线二等分试验评分均显著低于治疗前(P<0.05);治疗组治疗后平面图形临摹试验、画钟试验、线段划销试验、直线二等分试验评分均显著低于对照组(P<0.05)。两组治疗前CBS评分、MMSE评分、FMA评分、MBI评分比较差异无统计学意义(P>0.05);两组治疗后的CBS评分均显著低于治疗前(P<0.05),MMSE评分、FMA评分、MBI评分均显著高于治疗前(P<0.05);治疗组治疗后CBS评分显著低于对照组(P<0.05),MMSE评分、FMA评分、MBI评分均显著高于对照组(P<0.05)。治疗组患者内在满意度、总满意度均显著高于对照组(P<0.05),但两组患者的外在满意度比较差异无统计学意义(P>0.05)。结论彩色硅胶罐联合运动疗法治疗卒中后偏侧忽略的临床效果较单独运动疗法好。

Objective To observe the clinical efficacy of colored silica gel can plus exercise therapy in treating unilateral neglect after stroke.Method Sixty patients with post-stroke unilateral neglect were randomized into two groups,with 30 cases in each group.The treatment group was intervened by colored silica gel can plus exercise therapy,while the control group was intervened by exercise therapy alone.The unilateral spatial neglect parameters,Catherine Bergego Scale(CBS)score,Mini-Mental State Examination(MMSE)score,FugL-Meyer Assessment(FMA)score,Modified Barthel Index(MBI)score and satisfaction degree in the two groups were statistically analyzed.Result There were no significant differences in the planar pattern copying test(PPCT),clock drawing test(CDT),line cancellation test(LCT)and bisection line task(BLT)scores between the two groups before treatment(P>0.05);the scores of PPCT,CDT,LCT and BLT dropped significantly after treatment in both groups(P<0.05);after treatment,the PPCT,CDT,LCT and BLT scores in the treatment group were significantly lower than those in the control group(P<0.05).Prior to treatment,there were no significant differences in the CBS,MMSE,FMA and MBI scores between the two groups(P>0.05);the CBS score dropped significantly(P<0.05)and the MMSE,FMA and MBI scores increased significantly(P<0.05)after treatment in both groups;after treatment,the CBS score was significantly lower(P<0.05)and the MMSE,FMA and MBI scores were significantly higher(P<0.05)in the treatment group than in the control group.The internal satisfaction degree and total satisfaction degree in the treatment group were significantly higher than those in the control group(P<0.05),while there was no significant difference in the external satisfaction degree between the two groups(P>0.05).Conclusion Colored silica gel can plus exercise therapy can produce more significant clinical efficacy than exercise therapy alone in treating post-stroke unilateral neglect.
