针刺联合归肾丸对肾阳虚男性不育患者精液常规指标和性功能的影响

Effects of Acupuncture Combined with Gui Shen Wan on Semen Routine Indexes and Sexual Function in Male Patients with Infertility Due to Kidney-yang Deficiency

目的观察针刺联合归肾丸对肾阳虚男性不育患者精液常规指标水平及性功能的影响。方法选择96例肾阳虚型男性不育患者作为研究对象,采用随机数字表法分为观察组和对照组,每组48例。对照组患者给予归肾丸治疗,观察组在对照组基础上给予针刺治疗。于治疗前后对两组患者中医症候进行评分,测定精液量、精子密度、存活率、前向运动率精液常规指标,采用中国早泄患者性功能评价表(CISFPE)、勃起功能国际问卷(IIEF-5)进行性功能评价,评价治疗疗效及安全性。结果观察组和对照组患者总有效率分别为97.9%、91.7%,组间比较差异无统计学意义(P>0.05);两组患者治疗后肾阳虚各症候得分均明显降低(P<0.05),观察组患者腰膝酸软、性欲减退、夜尿频多及总分低于对照组(P<0.05);两组患者治疗后精子密度、存活率及前向运动率升高(P<0.05),观察组优于对照组(P<0.05);治疗后观察组CISFPE评分优于对照组(P<0.05)。结论针刺联合归肾丸治疗可提高肾阳虚男性不育患者精液质量,提高性功能,疗效确切。

Objective To observe the effects of acupuncture plus Gui Shen Wan on semen routine indexes levels and sexual function in male patients with infertility due to kidney-yang deficiency.Method Ninety-six male patients with infertility due to kidney-yang deficiency were recruited as subjects and divided into an observation group and a control group using the random number table method,with 48 cases in each group.Patients in the control group were given Gui Shen Wan,while the observation group was given additional acupuncture treatment.Before and after treatment,the symptoms of traditional Chinese medicine(TCM)were scored,and the semen routine indexes including semen volume,sperm density,survival rate and forward motion rate were measured in the two groups of patients;the sexual function was evaluated using the Chinese Index of Sexual Function for Premature Ejaculation(CISFPE)and International Index of Erectile Function-5(IIEF-5).The treatment efficacy and safety were assessed.Result The total effective rate was 97.9%in the observation group versus 91.7%in the control group,and the between-group difference was statistically insignificant(P>0.05).The kidney-yang deficiency symptoms scores dropped significantly after treatment in both groups(P<0.05),and the scores of lumbar and knee flaccidity,libido and frequent nocturnal urination and the global score in the observation group were lower than those in the control group(P<0.05).The sperm density,survival rate and forward motion rate increased after treatment in both groups(P<0.05),and the observation group was superior to the control group(P<0.05).The CISFPE score in the observation group was better than that in the control group(P<0.05).Conclusion Acupuncture plus Gui Shen Wan can enhance the semen quality and sexual function,and thus produce valid efficacy in treating male patients with infertility due to kidney-yang deficiency.
