摘要
前庭神经鞘瘤起源于听神经前庭支神经鞘细胞,是桥小脑角区最常见的良性肿瘤之一,由于其特殊的解剖结构,术后极易发生神经功能障碍。术中神经电生理监测技术的出现和进展,可以及时发现术中危险操作,进而从功能层面及早地避免神经损伤,对于安全切除肿瘤、术中最大限度地保护脑干和神经功能以及术后结果的预判具有重要价值,已成为前庭神经鞘瘤手术中不可或缺的部分。目前,前庭神经鞘瘤生长特性的分子机制和生物学行为预测正在深入研究中,未来神经电生理监测技术与精准治疗将为前庭神经鞘瘤患者提供个体化治疗方案。
Vestibular schwannoma originates from the nerve sheath cells of vestibular branches of the auditory nerve.It is one of the most common benign tumors in the cerebellopontine angle area.Due to its special anatomical structure,it is very easy to have neurological dysfunction after surgery.The emergence and progress of intraoperative neural electrophysiological monitoring technology,enables the in-time discovery of intraoperative risky operation,to avoid the nerve injury from the function level as soon as possible,which has great value for the safe removal of the tumor,maximum intraoperative protection of brain stem and neural function and judgment of postoperative result,and has become an integral part of vestibular schwannoma surgery.At present,the molecular mechanism and biological behavior prediction of vestibular neuroma growth characteristics are under in-depth study.In the future,the neuroelectrophysiological monitoring technology and precision therapy will provide individualized treatment for the patients.
