妊娠早期维生素A缺乏与血常规参数相关性分析

摘要 目的了解妊娠早期孕妇维生素A缺乏现状,探讨维生素A缺乏对血常规参数的影响。方法选取2016年11月至2018年5月于本院建档产检的孕妇1 270名,抽取空腹静脉血检测维生素A浓度及血常规,通过SPSS软件分析妊娠早期维生素A缺乏情况,了解血清维生素A缺乏对血常规参数的影响。结果妊娠早期维生素A缺乏的比例为15.43%,高龄产妇(≥35岁)与非高龄产妇(<35岁)缺乏率比较差异无统计学意义(x^2=0.433,P=0.510)。维生素A缺乏组孕妇的LYMPH#、LYMPH%、RBC、HGB、HCT、MCHC和PLT等参数均低于正常组(t=2.035、2.422、4.979、6.456、5.832、4.521、2.476,P<0.05),而MONO#、MONO%、RDW-CV、MPV和PDW等参数均高于正常组,差异具有统计学意义(t=-4.807、-5.901、-5.687、-4.375、-3.883,P<0.05)。结论本院妊娠早期孕妇维生素A缺乏的比例仍较高,并可导致相应血常规参数改变。因此,孕期有必要进行维生素A浓度的监测,及时发现维生素A缺乏的孕妇,并予以补充纠正,以保证孕妇及胎儿的健康。 Objective To study the incidence of vitamin A deficiency in first trimester and the effect of vitamin A deficiency on blood routine parameters. Methods 1 270 early pregnant women who were filed for maternity check-ups in our hospital from November 2016 to May 2018 were selected. All pregnant women were tested for vitamin A concentration and blood routine by fasting venous blood. All the data were analyzed by SPSS software to clarify the incidence of vitamin A deficiency in early pregnancy, and to study the influence of vitamin A deficiency on blood routine parameters. Results The incidence of vitamin A deficiency in first trimester was 15.43%, and there was no statistically significant difference between older women(age≥35 years) and non-older women(age<35 years). The parameters such as LYMPH#, LYMPH %, RBC, HGB, HCT, MCHC and PLT in vitamin A deficiency group were lower than normal group(t=2.035, 2.422, 4.979, 6.456, 5.832, 4.521 6.456, P<0.05), while MONO#,MONO %, RDW-CV, MPV and PDW were higher than normal group(t=-4.807,-5.901,-5.687,-4.375,-3.883, P<0.05), the difference was statistically significant. Conclusion The incidence of vitamin A deficiency in early pregnany in our hospital is still high. Vitamin A deficiency can affect hematopoiesis. Therefore, it is necessary to monitor the concentration of vitamin A during pregnancy and correct vitamin A deficiency in pregnant women, so as to ensure the health of pregnant women and fetuses.
