摘要
目的探讨肝脏MRI增强动脉期采集三个时相的扫描模式的可行性。方法收集298例肝脏MRI增强检查影像资料,根据动脉期采集时相的不同分成单动脉时相组和三动脉时相组。2名医师进行阅片评分,评分内容1)图像总体质量;2)有无呼吸运动伪影;3)有无SENSE伪影/条纹状伪影;4)肝脏边缘和血管锐利度;5)局灶性病灶的显示情况;6)早、中、晚三个动脉时相的显示情况。采用SPISS V11对上述两组评分结果进行统计分析,P<0.05为有意义。结果三动脉时相组中显示动脉晚期的受检者人数A和B医师均113例(82.48%),单动脉时相组中显示动脉晚期的受检者人数为A医师70例(43.48%)、B医师80例(49.69%)。结论采集三个动脉时相的扫描较传统的单动脉时相扫描可以显著提高最佳动脉晚期的准确显示率。
Objective To investigate the clinical value of three phases of hepatic 3.0T MR enhancement scanning in arterial phase.Methods 298 cases of liver MRI enhancement were collected and divided into single artery phase group(161 cases)and three artery phase group(137 cases)according to the different time phases of MR enhancement in arterial phase.Two doctors with more than five years experience in liver MRI diagnosis in our hospital independently scored the arterial phase images of the two groups on PACS.The scoring contents were as follows 1)Overall image quality;2)Respiratory motion artifacts;3)SENSE artifacts/Streak artifacts;4)Liver edge sharpness&hepatic vessel clarity;5)Conspicuity of focal liver lesions;and 6)Timing of arterial phase.SPISS V11 was used to analyze the scores of the two groups.The data of the two scorers were analyzed by paired sample t test,with statistical significance(P<0.05).Results The overall image quality,respiratory motion artifacts,SENSE artifacts/Streak artifacts,liver edge sharpness&hepatic vessel clarity of the three-artery phase group were similar to those of the single-artery phase group,without significant difference(P>0.05).The single-artery phase group scored l was significantly lower than the three-artery phase group in the display of focal lesions(P<0.001).Among the 137 patients in the three-artery phase group,113(82.48%)were accurately detection for late arterial phase both by scored 1 and 2,only 70(43.48%)scored 1 and 80(49.69%)scored 2 in the single-artery phase group by comparison.Conclusion The scanning of the three arteries time phases on the 3.0T MR equipment with one-breath-hold can obviously increase the best accurate display rate of the late arterial phase compared with the traditional single artery time phase scanning.
