三维超声测量胎儿脑干小脑蚓部夹角和脑干小脑幕夹角的应用价值 被引量:2

摘要 目的探讨三维超声VCI-C技术测量胎儿脑干-小脑蚓部夹角(BV角)和脑干小脑幕夹角(BT角)的重复性及一致性。方法选择我院行产前超声检查的120例胎儿做为观察对象并列为对照组,18例经MRI证实后颅窝畸形胎儿作为异常组,两组胎儿均采用三维超声VCI-C技术测量胎儿BV角和BT角,比较异常组与正常组之间的差异;并判断该技术测量的重复性及一致性。结果孕20~34周正常胎儿的BV角、BT角与孕周无相关性(P>0.05);采用三维超声VCI-C技术测量胎儿BV角、BT角的可重复性及一致性较高(ICC均>0.9);正常组胎儿的BV角、BT角均明显小于异常组(P<0.05)。结论三维超声VCI-C技术测量胎儿BV角和BT角技术可行,值得临床应用。 Objective To investigate the repeatability and consistency of three-dimensional ultrasound VCI-C technique in measuring the angles of brainstem cerebellar vermis(BV)and brainstem tentorium(BT),and to analyze the diagnostic value of this technique for posterior cranial fossa deformity.Methods From September 2017 to September 201,120 normal fetuses who underwent prenatal ultrasound examination in our hospital were selected as the study objects.18 fetuses with deformity of posterior cranial fossa confirmed by MRI were used as the abnormal group.The BV angle and BT angle of brainstem were measured by three-dimensional ultrasound VCI-C technology in both groups,and the difference between the abnormal fetuses and the normal fetuses was compared.The repeatability and consistency of the measurement were judged.Results There was no correlation between bv angle,BT angle and gestational age(P>0.05);the repeatability and consistency of BV angle and BT angle measured BV three-dimensional ultrasound VCI-C were higher(ICC>0.9);the BV angle and BT angle of normal group were significantly lower than those of abnormal group(P<0.05).Conclusion It is feasible,reproducible and consistent to measure BV angle and BT angle of fetal brainstem with three-dimensional ultrasound VCI-C,which can be used as a reference for clinical diagnosis of posterior cranial fossa deformity.
