冠脉旁路移植术后心房颤动患者f波分形维数与f波形态、左房内径大小、心房颤动时间及心房颤动频率相关性研究

Correlation between fractal dimension of f-wave and shape of f-wave,left atrial diameter,atrial fibrillation duration and atrial fibrillation frequency in patients with atrial fibrillation after coronary artery bypass grafting

目的探讨冠脉旁路移植术后心房颤动患者f波分形维数(D值)与f波形态、左房内径大小、心房颤动时间及心房颤动频率的相关性。方法回顾性分析北部战区总医院2019年1—7月收治的单纯冠脉旁路移植术后发生心房颤动的121例患者的临床资料。心脏超声测量左房内径大小,长程动态心电图采集f波形态、心房颤动时间及心房颤动频率,采用斯皮尔曼双变量分析法分析f波D值与f波形态、左房内径大小、心房颤动时间及心房颤动频率相关性。结果f波D值越大,f波形态的不规则度及复杂度越高;反之,则相对规整和简单。f波D值为1.008905977~1.525847241,平均(1.116257326±0.103743945)。f波D值与左房内径大小呈高度正相关(ρ=0.858,P<0.05);与心房颤动时间呈中度正相关(ρ=0.430,P<0.05);与心房颤动频率无明显相关性(ρ=-0.021,P>0.05)。结论冠脉旁路移植术后心房颤动患者f波D值可反映f波形态的粗糙及复杂程度;在一定范围内,其还能够敏感地描述患者的左房内径大小,并对心房颤动时间具有预测性。

Objective To investigate the correlation between the fractal dimension of f-wave(D value)and the f-wave shape,the size of left atrial diameter,the atrial fibrillation(AF)duration and AF frequency in patients with AF after coronary artery bypass grafting.Methods A retrospective study was performed on 121 cases of patients with AF who were admitted and underwent coronary artery bypass grafting from January to July 2019.The left atrial diameter was measured by echocardiography,and the f-wave shape,AF duration and AF frequency were collected by long range dynamic electrocardiogram.The correlation between f-wave D value and f-wave shape,left atrial diameter,AF duration and AF frequency was analyzed by Spearman bivariate analysis.Results The larger the D value of fwave,the higher the irregularity and complexity of the shape of f-wave.Conversely,it was relatively neat and simple.The D value of fwave was 1.008905977~1.525847241,with an average(1.116257326±0.103743945).D value of f-wave was highly positively correlated with left atrial diameter(ρ=0.858,P<0.05).D value had a moderate positive correlation with AF duration(ρ=0.430,P<0.05).D value had no significant correlation with AF frequency(ρ=-0.021,P>0.05).Conclusion D value of f-wave in patients with atrial fibrillation after coronary artery bypass grafting can reflect the roughness and complexity of f-wave morphology.In a certain range,it can also sensitively describe the left atrial diameter of patients and predict the duration of atrial fibrillation.
