超声引导下导丝定位在触诊阴性的BI-RADS 4类及以上乳腺病变中的应用 被引量:1

摘要 目的:评价在触诊阴性的BI-RADS 4类及以上乳腺病变中,超声引导下导丝定位应用效果。方法:抽取40例本院收治的女性患者开展研究,研究时段:2018年1月—2020年5月。40例患者,均经触诊为阴性,超声检查为BI-RADS 4类及以上病变。术前,在超声引导下进行导丝定位,选择好穿刺点,术后沿导丝将病灶切除。结果:40例患者,共检出乳腺病灶42枚,经超声引导下导丝定位,成功率100%,所有病灶全部手术切除,病理检查证实,恶性病例7例,良性病例35例。良性患者,开展手术切除;恶性患者根据病情与患者所需,开展针对性治疗,即行保乳术者6例,行改良根治术者1例。且在恶性患者手术治疗中,没有因穿刺点的选择而影响手术切口。结论:在触诊阴性的BI-RADS 4类及以上乳腺病变中,超声引导下导丝定位可以对病灶进行准确定位,选择相应的进针点,快速切除病灶,且不会影响恶性患者切口的选择,值得临床借鉴。 Objective To evaluate the effect of ultrasound-guided guide wire localization in palpation negative BI-RADS 4 and above breast lesions.Methods 60 female patients in our hospital were selected for the study from January 2018 to May 2020.40 patients were negative by palpation,and BI-RADS 4 and above lesions were detected by ultrasonography.Before the operation,the guide wire was guided by ultrasound to locate the lesions,and the puncture point was selected.After the operation,the lesions were removed along the guide wire.Results 42 breast lesions were detected in 40 patients.Under the guidance of ultrasound,the localization rate of the guide wire was 100%.All the lesions were removed by operation and confirmed by pathological examination.There were 7 malignant cases and 37 benign cases.According to the condition and needs of patients,targeted treatment was carried out in 6 patients with breast conserving operation and 1 patients with modified radical operation.And in the surgical treatment of malignant patients,there is no influence on the incision due to the selection of puncture point.Conclusion In BI-RADS 4 and above breast lesions with negative palpation,ultrasound-guided guide wire positioning can accurately locate the lesions,select the corresponding needle entry point,and quickly remove the lesions,without affecting the choice of incision for malignant patients,which is worthy of clinical reference.
