当归饮子加减治疗皮肤瘙痒症的Meta分析

Meta Analysis of the Treatment of Skin Pruritus with Modified Chinese Angelica Cold Decoction

目的系统评价当归饮子加减治疗皮肤瘙痒症的临床疗效。方法计算机检索Pubmed,The Cochrane Library,Embase,中国期刊全文数据库,维普数据库和万方数据库,检索时间限定为建库至2018年7月17日,检索所有当归饮子加减治疗皮肤瘙痒症的随机对照试验。由两位评价者根据Cochrane系统评价手册独立对纳入研究的质量进行严格评价和有效信息提取后,采用RevMan5.3软件进行Meta分析。结果最终纳入9项研究,共866例患者。Meta分析结果显示:与常规西药及中成药治疗相比,当归饮子加减治疗皮肤瘙痒症可显著提高临床综合疗效[OR=3.93,95%CI(2.66,5.79),P<0.00001]。"漏斗图"分析结果显示不对称,可能存在潜在发表偏倚。结论当归饮子加减治疗皮肤瘙痒症临床疗效优于常规西药及中成药治疗。由于所纳入的研究方法学和报告质量较低且样本量较少,故当归饮子加减治疗皮肤瘙痒症的疗效仍需要多中心、大样本、高质量的随机对照试验加以验证。

Objective:To systematically evaluate the clinical effect of modified Chinese Angelica Cold Decoction on the treatment of skin pruritus.Methods:By computer retrieval of Pubmed,The Cochrane Library,Embase,Chinese Journal Full-text Database,Vip Database and Wanfang Database and with the search time limitation to the establishment of the database until July 17,2018,all the randomized controlled trials for skin pruritus treatment by modified Chinese Angelica Cold Decoction were searched.After the two reviewers independently strictly evaluated the quality of the included studies and extracted effective information according to the Cochrane System Evaluation Manual,RevMan5.3 software was used to make Meta analysis.Results:9 items of the studies were finally included,a total of 866 patients altogather.Meta analysis results showed that when compared with conventional western medicine and Chinese patent medicine treatment,the modified Chinese Angelica Cold Decoction for the treatment of skin pruritus could significantly improve the clinical comprehensive effect(OR=3.93,95%CI(2.66,5.79),P<0.00001).Funnel Chart analysis results showed asymmetry and there may be potential publication bias.Conclusion:The clinical effect of Chinese Angelica Cold Decoction on the treatment of skin pruritus is better than that of conventional western medicine and Chinese patent medicine.Due to the low quality of the research methodology and reports included and the small sample size,the curative effect of the modified Chinese Angelica Cold Decoction on the treatment of skin pruritus still needs to be verified by a multi-center,large sample,high-quality randomized controlled trial.
