摘要
目的探讨腹主动脉瘤(abdominal aortic aneurysm,AAA)在腹主动脉瘤的表达,寻找预防及治疗AAA的新靶点。方法本研究纳入2017年9月至2019年11月住院及门诊AAA患者20例和体检健康人20例为研究对象,采用酶联免疫吸附法(ELISA)测定血清Fibulin-3、基质金属蛋白酶(matrix metalloproteinase,MMPs)水平;同时收集血管外科AAA病例组织标本10例,器官移植外科器官捐献者正常腹主动脉组织标本10例为对照组,采用荧光定量PCR和WB测定血管壁中Fibulin-3的表达水平。结果血清及血管壁中Fibulin-3表达水平在AAA组均明显低于健康对照组,差异有统计学意义(P <0.01);而血清中MMP-2及MMP-9表达水平在AAA组均明显高于健康对照组,差异有统计学意义(P <0.01)。血清Fibulin-3ROC曲线分析结果提示,其具有较高的预测价值。结论腹主动脉瘤患者血清及血管壁中Fibulin-3水平降低而血清MMP-2及MMP-9表达升高,提示AAA的发生与Fibulin-3有关,且Fibulin-3可能可作为AAA的一个预测指标。
Objective Abdominal aortic aneurysm(AAA) is a severe cardiovascular disease.To explore the expression of Fibulin-3 in abdominal aortic aneurysms and to find new targets for prevention and treatment of AAA.Methods This study included 20 patients with inpatient and outpatient AAA and 20 healthy people from September 2017 to November 2019.The serum Fibulin-3,Matrix metalloproteinase(MMPs) level in serum was determined by enzyme-linked immunosorbent assay(ELISA).At the same time,10 AAA tissue samples from vascular surgery were collected,and 10 normal abdominal aortic tissue samples from organ donors for organ transplant surgery were used as a control group.Fibulin-3 level in the vascular wall was determined using flurogenic quantitative polymerase chain reaction(qPCR) and Western Blot(WB).Results Fibulin-3 levels in the serum and the blood vessel wall were significantly lower in the AAA group than that of the healthy control group,and the difference were statistically significant(P <0.01).The expression levels of MMP-2 and MMP-9 in the serum were significantly higher in the AAA group compared with the control group,the difference was statistically significant(P <0.01).The results of serum Fibulin-3 ROC curve analysis suggest that it has high predictive value.Conclusions Fibulin-3 levels in serum and vascular wall of patients with abdominal aortic aneurysms are reduced,while serum MMP-2 and MMP-9 expressions are increased.We suggest that the occurrence of AAA is related to Fibulin-3,and it may be used as a predictor of AAA.Whether increasing Fibulin-3 levels is beneficial to inhibit the occurrence of AAA or become a new therapeutic target is worth further research.
