摘要
目的探讨五志过极护理在体外反搏治疗缺血性脑卒中(CIS)后睡眠障碍(SD)患者中的应用效果。方法选取2018年7月~2019年6月我院收治的120例CIS后SD患者作为研究对象,按照随机抽签法将其分为对照组(n=60)与研究组(n=60)。两组均实施体外反搏治疗方法,对照组采用常规内科护理方法,研究组采用五志过极护理方法。比较两组护理前后的匹兹堡睡眠质量指数(PSQI)各因子评分及总分、健康SF-36量表(中文版)评分。结果两组护理前的睡眠时长评分、入睡时长评分、日间功能评分、睡眠质量评分、睡眠效率评分、睡眠障碍评分、总分比较,差异无统计学意义(P>0.05);研究组护理后的睡眠时长评分为(5.15±0.45)分,高于对照组[(3.93±0.65)分],差异有统计学意义(P<0.05);研究组护理后的入睡时长评分为(1.15±0.33)分,日间功能评分为(6.75±0.60)分,睡眠质量评分为(1.55±0.25)分,睡眠效率评分为(0.60±0.25)分,睡眠障碍评分为(0.85±0.20)分,总分为(15.85±0.65)分,均低于对照组[(1.45±0.41)、(7.95±0.40)、(1.95±0.60)、(1.05±0.50)、(1.37±0.55)、(17.85±1.54)分],差异有统计学意义(P<0.05);两组护理前的总体健康评分、生理功能评分、生理职能评分、躯体疼痛评分、活力评分、社会功能评分、情感职能评分、精神健康评分比较,差异无统计学意义(P>0.05);研究组护理后的总体健康评分为(74.65±12.30)分,生理功能评分为(72.65±13.50)分,生理职能评分为(66.75±10.45)分,躯体疼痛评分为(69.85±11.50)分,活力评分为(80.65±14.50)分,社会功能评分为(81.65±14.30)分,情感职能评分为(90.35±12.50)分,精神健康评分为(76.85±13.50)分,均高于对照组[(62.45±10.01)分、(66.35±10.30)分、(58.45±8.70)分、(63.95±10.50)、(71.70±12.30)、(74.65±12.40)、(77.75±13.50)、(66.35±12.50)分],差异有统计学意义(P>0.05)。结论CIS后SD患者实施五志过极护理结合体外反搏可有效改善其PSQI各因子及总分、生活质量评分。
Objective To explore the application effect of nursing on the five excessive emotions in the external counterpulsation treatment of patients with sleep disorder(SD)after ischemic stroke(CIS).Methods Totally 120 patients with SD after CIS treated in our hospital from July 2018 to June 2019 were selected as research subjects.They were divided into control group(n=60)and study group(n=60)by randomly drawing lots.The two groups were both treated with external counterpulsation.Meanwhile,the patients in the control group were treated with conventional nursing,and the patients in the study group were provided with the five excessive emotions nursing.The Pittsburgh sleep quality index(PSQI)factors and total scores,and the score of healthy SF-36 scale(Chinese version)were compared between the two groups before and after nursing.Results The sleep duration score,sleep latency score,daytime function score,sleep quality score,sleep efficiency score,sleep disorder score,and total score between the two groups had no significant differences(P>0.05).After nursing,the score of sleep duration in the study group was(5.15±0.45)points,which was higher than that of the control group of(3.93±0.65)points,the difference was statistically significant(P<0.05).After nursing,the sleep latency score,day function score,sleep quality score,sleep efficiency score,sleep disorder score and total score in the study group were(1.15±0.33),(6.75±0.60),(1.55±0.25),(0.60±0.25),(0.85±0.20)and(15.85±0.65)points,respectively,lower than those of the control group of(1.45±0.41),(7.95±0.40),(1.95±0.60),(1.05±0.50),(1.37±0.55)and(17.85±1.54)points,respectively,the differences were statistically significant(P<0.05).The overall health score,physiological function score,physiological role score,physical pain score,vitality score,social function score,emotional role score,and mental health score in the two groups before nursing had no significant differences(P>0.05).After treatment,the overall health score,physiological function score,physiological role score,physical pain score,vitality score,social function score,emotional role score,and mental health score in the study group were(74.65±12.30),(72.65±13.50),(66.75±10.45),(69.85±11.50),(80.65±14.50),(81.65±14.30),(90.35±12.50)and(76.85±13.50)points,respectively,higher than those of the control group of(62.45±10.01),(66.35±10.30),(58.45±8.70),(63.95±10.50),(71.70±12.30),(74.65±12.40),(77.75±13.50)and(66.35±12.50)points,respectively with statistical differences(P<0.05).Conclusion Implementing nursing on the five excessive emotions combined with external counterpulsation on patients with SD after CIS can effectively improve their PSQI factors,PSQI total score and quality of life score.
