腹腔镜结直肠癌根治术治疗结直肠癌患者的疗效与安全性

Efficacy and safety of laparoscopic radical resection for patients with colorectal cancer

目的观察评价分析腹腔镜结直肠癌根治术治疗结直肠癌(CRC)患者的效果。方法选择2017年2月至2018年2月本院收治的CRC患者98例,随机对照试验(RCT)分为对照组与观察组,各49例。对照组开腹CRC根治术治疗;观察组腹腔镜CRC根治术治疗,比较两组临床效果。结果观察组总有效率为97.96%,明显高于对照组的83.67%(P<0.05);观察组手术时间、术中出血量及住院时间均少于对照组(均P<0.05),而两组淋巴结清扫数差异无统计学意义(P>0.05);观察组肛门排气、腹胀持续、肠鸣音恢复、开始进食时间均明显少于对照组(均P<0.05);观察组术后总并发症发生率为6.12%,明显的低于对照组的20.41%(P<0.05)。结论腹腔镜CRC根治术治疗CRC患者效果确切,能够促进患者胃肠功能的恢复,有效缩短手术时间及住院时间,减少出血量,可明显降低对患者的创伤,大大减少了并发症发生率,值得临床大力推广应用。

Objective To observe and evaluate the effect of laparoscopic radical resection for patients with colorectal cancer(CRC).Methods Ninety-eight patients with CRC treated at our hospital from February,2017 to February,2018 were selected.The randomized controlled trial(RCT)was used to divided them into a control group and an observation group,with 49 cases in each group.The control group was treated with open CRC radical surgery,and the observation group with laparoscopic radical resection.The clinical effects of the two groups were compared.Results The total effective rate was 97.96%in the observation group,and was 83.67%in the control group(P<0.05).The operation time,intraoperative bleeding voume,and hospital stay in the observation group were less than those in the control group(all P<0.05).There was no statistical difference in lymph node dissection numbers between the two groups(P>0.05).The time to anal exhaustion,abdominal distension duration,time for recovery of bowel sounds,and time to start eating in the observation group were all significantly less than those in the control group(all P<0.05).The total incidence of postoperative complications was significantly lower in the observation group than in the control group(6.12%vs.20.41%;P<0.05).Conclusion Laparoscopic CRC radical surgery is effective in treating patients with CRC,and can promote the recovery of gastrointestinal function,effectively shorten the operation time and hospitalization time,and reduce the amount of bleeding,the trauma to patients,and the incidence of complications,so it is worthy of clinical promotion and application.
