电子支气管镜肺泡灌洗治疗小儿重症肺炎的效果观察 被引量:7

Effects of electronic bronchoscopy alveolar lavage on children with severe pneumonia

摘要 目的探讨电子支气管镜肺泡灌洗治疗重症肺炎患儿的效果。方法选取2016年1月至2019年8月于滨州市中心医院接受治疗的重症肺炎患儿100例,按照随机数字表法将其分为对照组与观察组,每组50例。对照组采用常规治疗,观察组采用常规治疗联合电子支气管镜肺泡灌洗治疗,对比两组症状恢复时间、血清炎症因子水平及血气指标。结果观察组治疗后发热、咳嗽、肺部啰音消失时间及呼吸困难缓解时间均短于对照组,差异有统计学意义(P<0.05);观察组治疗后血清白细胞介素-6[(25.15±3.33)pg/ml]、白细胞介素-10[(14.79±0.65)pg/ml]水平均低于对照组[(32.13±3.46)、(19.64±0.87)pg/ml],差异有统计学意义(P<0.05);观察组动脉血氧分压高于对照组,而动脉血二氧化碳分压、血乳酸水平均低于对照组,差异有统计学意义(P<0.05)。结论电子支气管镜肺泡灌洗治疗小儿重症肺炎效果较好,可改善炎症状态及血氧水平,缩短症状恢复时间。 Objective To investigate the therapeutic effects of electronic bronchoscope alveolar lavage on children with severe pneumonia.Methods A total of 100 children with severe pneumonia who received treatment in Binzhou Central Hospital from January 2016 to August 2019 were selected,and they were divided into control group and observation group according to random number table method,with 50 cases in each group.The control group received conventional treatment,while the observation group received conventional treatment combined with electronic bronchoscope alveolar lavage.The symptom recovery time,serum inflammatory factor level and blood gas indexes of the two groups were compared.Results After treatment,the appearance times of fever,cough,racquet and dyspnea in the observation group were shorter than those in the control group,and the difference was statistically significant(P<0.05).The levels of serum interleukin-6 and interleukin-10 of the observation group were(25.15±3.33)pg/ml and(14.79±0.65)pg/ml,respectively,lower than the(32.13±3.46)pg/ml and(19.64±0.87)pg/ml in the control group after treatment(P<0.05).Arterial partial blood oxygen pressure in the observation group was higher than that in the control group,while arterial partial blood carbon dioxide pressure and blood lactic acid levels were lower than those in the control group,with statistically significant differences(P<0.05).Conclusions Electronic bronchoscope alveolar lavage is effective in the treatment of severe pneumonia.
