心脉隆注射液静脉泵入对脓毒血症合并心衰患者血清PCT、BNP、NT-proBNP含量影响及临床意义

Influence of intravenous pumped Xinmailong Injection on levels of serum PCT,BNP and NT-proBNP in patients with sepsis complicated by heart failure and a study on clinical significance

目的探讨心脉隆注射液静脉泵入对脓毒血症合并心力衰竭(心衰)患者血清降钙素原(PCT)、脑钠肽(BNP)、氨基末端脑钠肽前体(NT-proBNP)含量影响及临床意义。方法选取2015年12月至2017年11月间于保定市第一中心医院收治的脓毒血症合并心衰患者100例,随机分为对照组和观察组两组,每组各50例。对照组采用抗感染和抗心衰治疗,观察组在对照组基础上加用心脉隆注射液治疗,2组均连续治疗7 d。比较两组患者心衰临床疗效(Lee氏心衰计分)及治疗前、后的心功能改善指标:左室射血分数、心房收缩期峰值血流速度的比值、心排血量(LVEF、E/A、CO)和血清学指标PCT、BNP、NT-proBNP。结果治疗后,两组的Lee氏心衰计分、心功能检测指标水平和血清学指标水平均较治疗前改善(P<0.05),观察组改善更为显著(P<0.05)。结论心脉隆注射液治疗脓毒血症合并心衰疗效较好,可明显下调患者血清PCT、BNP和NT-proBNP水平,降低炎症和改善心肌损伤。

Objective To investigate the influence of intravenous pumped Xinmailong Injection on levels of serum procalcitonin(PCT),brain natriuretic peptide(BNP)and N-terminal pro-brain natriuretic peptide(NT-proBNP)in patients with sepsis complicated by heart failure(HF)and study the clinical significance.Methods The patients(n=100)were chosen from the First Central Hospital of Baoding City from Dec.2015 to Nov.2017,and then divided randomly into control group and observation group(each n=50).The control group was given anti-infection and anti-HF treatment,and observation group was additionally given Xinmailong Injection for 7 d.The clinical efficacy on HF(Lee’s heart failure scoring),indexes of heart function including left ventricular ejection fraction(LVEF),ratio of atrial peak systolic velocity(E/A ratio)and cardiac output(CO),and serum indexes including PCT,BNP and NT-proBNP were compared between 2 groups.Results After treatment,Lee’s heart failure scores,indexes of heart function and serum indexes were all improved in 2 groups(P<0.05),which was more significant in observation group(P<0.05).Conclusion Xinmailong Injection has a higher curative effect on sepsis complicated by HF,and it can significantly down-regulate the levels of PCT,BNP and NT-proBNP and relief inflammation and myocardial injury.
