肺源性心脏病伴慢性呼吸衰竭患者无创呼吸机治疗依从性的影响因素分析及护理干预

Analysis of influencing factors of non-invasive ventilator treatment compliance and its nursing intervention in patients with cor pulmonale and chronic respiratory failure

目的探讨肺源性心脏病伴慢性呼吸衰竭患者无创呼吸机治疗依从性的影响因素及护理干预措施。方法选取2018年6月至2019年7月我院收治的慢性呼吸衰竭患者97例作为研究对象。通过问卷调查及访谈方式收集患者基本资料、临床资料及治疗依从性情况,单因素分析其治疗依从性的相关影响因素,通过多元线性回归分析确定其独立影响因素。结果结果显示肺源性心脏病患者无创呼吸机治疗依从性处于较低水平。单因素分析显示,家庭收入、文化程度、呼吸机使用次数、呼吸机使用了解程度、呼吸机使用效果、呼吸机使用舒适度、并发症、焦虑程度、自我管理及自我效能为肺源性心脏病患者无创呼吸机治疗依从性的相关影响因素(P<0.05);多元线性回归分析显示,呼吸机了解程度、呼吸机使用舒适度、并发症及焦虑程度为肺源性心脏病患者无创呼吸机治疗依从性的独立影响因素(P<0.05)。结论呼吸机了解程度、呼吸机使用舒适度、并发症及焦虑程度为肺源性心脏病患者无创呼吸机治疗依从性的独立影响因素,需根据以上影响因素制定相应的护理措施以提高肺源性心脏病患者无创呼吸机治疗依从性。

Objective To explore the influencing factors of non-invasive ventilator treatment compliance and its nursing intervention measures in patients with cor pulmonale and chronic respiratory failure.Methods To select 97 cases of patients with chronic respiratory failure admitted to our hospital from June 2018 to July 2019 as the study objects.The basic data,clinical data,and treatment compliance of patients were collected through questionnaire surveys and interviews,and the related influencing factors of their treatment compliance were analyzed by single factor,and the independent influencing factors were determined by multiple linear regression analysis.Results The results showed that the patients with cor pulmonale had a low level of non-invasive ventilator treatment compliance.Univariate analysis showed that the family income,education level,number of times of using ventilator,understanding degree of using ventilator,effects of using ventilator,comfort of using ventilator,complications,degree of anxiety,self-management and self-efficacy were the relevant factors affecting non-invasive ventilator treatment compliance in patients with cor pulmonale(P<0.05).Multiple linear regression analysis showed that the understanding degree of ventilator,comfort of using ventilator,complications,and degree of anxiety were the independent factors affecting non-invasive ventilator treatment compliance in patients with cor pulmonale(P<0.05).Conclusion The understanding degree of ventilator,comfort of using ventilator,complications,and degree of anxiety were the independent factors affecting non-invasive ventilator treatment compliance in patients with cor pulmonale,it was necessary to formulate corresponding nursing measures according to the above influencing factors to improve the non-invasive ventilator treatment compliance in patients with cor pulmonale.
