3D打印技术在疑难复杂掌指骨粉碎性骨折中的临床应用研究

摘要 目的探讨3D打印技术在疑难复杂掌指骨粉碎性骨折患者中的临床应用价值。方法60例疑难复杂掌指骨粉碎性骨折患者,依照治疗环节所用技术不同分为参照组和实验组,各30例。参照组施以常规技术,实验组施以3D打印技术,对比两组临床疗效、术中操作时间、X线透视次数、出血量、关节功能评分、手术并发症发生情况和患者满意度。结果实验组的优良率96.67%高于参照组的70.00%,差异具有统计学意义(P<0.05)。实验组的术中操作时间(2.2±0.1)h短于参照组的(4.6±0.2)h,差异具有统计学意义(P<0.05);实验组的X线透视次数、出血量分别为(2.4±0.1)次、(30.4±5.1)ml,少于参照组的(4.8±0.2)次、(129.8±7.0)ml,差异具有统计学意义(P<0.05)。实验组的关节功能评分(9.0±0.5)分、患者满意度(9.5±0.1)分均高于参照组的(7.8±1.1)、(7.3±0.2)分,手术并发症(0.4±0.2)项少于参照组的(4.2±0.3)项,差异具有统计学意义(P<0.05)。结论3D打印技术在疑难复杂掌指骨粉碎性骨折患者中的治疗效果确切,具有较高的临床有效性和安全性,对提高手术精准度、成功率、减轻患者痛苦效果显著,值得临床大力实施推广。 Objective To discuss the clinical value of 3 D printing technology in difficult and complicated comminuted periarticular metacarpal and phalangeal fractures.Methods A total of 60 patients with difficult and complicated comminuted periarticular metacarpal and phalangeal fractures were divided into control group and experimental group according to the technical differences in the treatment,with 30 cases in each group.The control group received common technology,and the experimental group received 3 D printing technology.The clinical efficacy,operation time,number of X-ray fluoroscopy,blood loss,joint function score,occurrence of surgical complications and satisfaction degrees were compared between the two groups.Results The excellent rate 96.67%of the experimental group was higher than that of the control group 70.00%,and the difference was statistically significant(P<0.05).The operation time(2.2±0.1)h of the experimental group was shorter than that of the control group(4.6±0.2)h,and the difference was statistically significant(P<0.05).The number of X-ray fluoroscopy and blood loss of the experimental group was(2.4±0.1)times and(30.4±5.1)ml respectively,which was less than that of the control group(4.8±0.2)times and(129.8±7.0)ml,and the difference was statistically significant(P<0.05).The joint function score(9.0±0.5)points and satisfaction degrees(9.5±0.1)points of the experimental group was higher than that of the control group(7.8±1.1)and(7.3±0.2)points,and surgical complications(0.4±0.2)types was less than that of the control group(4.2±0.3)types,and the difference was statistically significant(P<0.05).Conclusion 3 D printing technology has a definite therapeutic effect in patients with difficult and complicated comminuted periarticular metacarpal and phalangeal fractures,and has high clinical effectiveness and safety.It has a significant effect on improving the accuracy of surgery,success rate,and reducing the pain of patients.It is worthy of clinical promotion.
