捋顺扣挤正骨法联合中药内服及熏洗治疗老年桡骨远端骨折的临床效果

Clinical effect of Lyushun Kouji Bone-Setting Manipulation combined with oral administration of traditional Chinese medicine and fumigation-washing therapy on senile distal radius fracture

目的观察捋顺扣挤正骨法联合中药内服及熏洗治疗老年桡骨远端骨折的疗效及安全性。方法将150例老年桡骨远端骨折患者根据随机数字表法分为对照组与观察组,各75例。两组患者均接受捋顺扣挤正骨法治疗,观察组在此基础上联合中药内服及中药熏洗治疗,疗程均为2个月。比较两组患者疼痛缓解时间、患处肿痛消失时间、骨折愈合时间以及治疗期间不良反应发生情况;分别于治疗前、治疗结束时,采用关节角度测量仪测量患者腕关节活动度,治疗结束时采用Dienst腕关节评分量表评估两组腕关节功能改善情况。结果观察组疼痛缓解时间、患处肿痛消失时间及骨折愈合时间均短于对照组,治疗结束时,腕关节活动度及腕关节功能优良率均高于对照组(均P<0.05),但治疗期间两组患者不良反应发生率差异无统计学意义(P>0.05)。结论捋顺扣挤正骨法联合中药内服及熏洗治疗老年桡骨远端骨折疗效确切,骨折愈合时间短,腕关节活动度与功能改善明显,且不良反应少。

Objective To observe the efficacy and safety of Lyushun Kouji Bone-Setting Manipulation combined with oral administration of traditional Chinese medicine(TCM)and fumigation-washing therapy for treating senile distal radius fracture.Methods A total of 150 elderly patients with distal radius fracture were divided into control group and observation group according to the random number table method,with 75 cases in each group.Both groups were treated with Lyushun Kouji Bone-Setting Manipulation,based on which the observation group received combined oral administration of TCM and fumigation-washing therapy,and the course of treatment was two months in both groups.Pain relief time,duration for recovery from swelling and pain at affected sites,fracture healing time,and incidence of adverse reactions during treatment were compared between the two groups.Before and at the end of treatment,range of wrist joint motion was measured by joint angle measuring instrument.At the end of treatment,the improvement of wrist joint function was evaluated by the Dienst wrist joint score scale in the two groups.Results Compared with the control group,the observation group achieved shorter pain relief time,shorter duration for recovery from swelling and pain at affected sites,shorter fracture healing time,increased range of wrist joint motion at the end of treatment,and higher excellent and good rate of wrist joint function at the end of treatment(all P<0.05),and there was no statistically significant difference between the two groups in incidence rate of adverse reactions during treatment(P>0.05).Conclusion Lyushun Kouji Bone-Setting Manipulation combined with oral administration of TCM and fumigation-washing therapy has definite efficacy for treating senile distal radius fracture,achieving shorten fracture healing time,notably improved range of motion and function of wrist joint and fewer adverse reactions.
