过敏性紫癜性肾炎患儿的红细胞分布宽度及其临床意义

Red blood cell distribution width and its clinical significance in children with Henoch-Sch nlein purpura nephritis

目的探讨过敏性紫癜性肾炎(HSPN)患儿的红细胞分布宽度(RDW)及其临床意义。方法纳入过敏性紫癜(HSP)患儿38例(HSP组),HSPN患儿58例(HSPN组)和健康儿童60例(对照组)。检测并比较3组受试者的血常规。比较不同病情程度HSPN患儿的RDW,并应用受试者工作特征(ROC)曲线分析RDW对HSPN严重程度的预测价值。结果HSP组和HSPN组白细胞计数、中性粒细胞计数均高于对照组,并且血红蛋白低于对照组;HSPN组血红蛋白高于HSP组,RDW高于对照组(均P<0.05)。有新月体形成的HSPN组患儿RDW高于无新月体形成组患儿(P<0.05)。RDW值为13.45%时,RDW诊断有新月体形成的HSPN的ROC曲线下面积为0.723,灵敏度为69.7%,特异性为73.8%。结论HSPN患儿的RDW高于健康儿童,RDW对HSPN的诊断及病情严重程度评估均有一定价值。

Objective To investigate red blood cell distribution width(RDW)and its clinical significance in children with Henoch-Sch nlein purpura nephritis(HSPN).Methods Thirty-eight children with Henoch-Sch nlein purpura(HSP)were enrolled as HSP group,58 children with HSPN as HSPN group,and 60 healthy children as control group.Blood routine test was conducted and compared in subjects of the three groups.RDW was compared among children with HSPN of different severities,in addition,receiver operating characteristic(ROC)curve was used to analyze the predictive value of RDW for severity of HSPN.Results Compared with the control group,the HSP group and the HSPN group had higher WBC count and neutrophil count,as well as lower hemoglobin;the HSPN group exhibited higher hemoglobin than the HSP group and higher RDW than the control group(all P<0.05).Children with crescent formation in the HSPN group displayed higher RDW than children without crescent formation(P<0.05).The area under the ROC curve of RDW was 0.723,when RDW value was 13.45%,in diagnosing the presence of crescent in HSPN,with a sensitivity of 69.7%and a specificity of 73.8%.Conclusion HSPN children experience elevated RDW than healthy children,and RDW has a certain value for HSPN diagnosis and disease severity assessment.
