龙骨小麦疏肝汤对肝郁脾虚型甲状腺功能亢进患者复发及FT3、FT4的影响

Effect of Longgu Xiaomai Shugan Decoction on Recurrence,FT3 and FT4 in Patients with Liver Depression and Spleen Deficiency Hyperthyroidism

目的探究龙骨小麦疏肝汤对肝郁脾虚型甲状腺功能亢进患者复发及FT3、FT4的影响。方法纳入2016年1月—2018年7月收治的62例甲状腺功能亢进患者,按随机数字表法将其分为对照组(31例)和观察组(31例)。对照组给予甲巯咪唑片治疗,观察组在此基础上给予龙骨小麦疏肝汤治疗,2组均连续治疗3个月并随访1年。统计2组治疗后临床疗效;比较2组患者治疗前后中医证候积分、甲状腺激素水平,比较2组治疗期间不良反应发生情况及随访期间复发情况。结果观察组临床总有效率为96.77%,明显高于对照组的74.19%(P<0.05);观察组复发率为7.14%,低于对照组的33.33%(P>0.05)。2组心悸不定、手指震颤、倦怠乏力及烦躁易怒等中医证候评分及血清FT3、FT4、TT3、TT4水平均较治疗前降低,观察组低于对照组(P<0.05),2组血清TSH水平较治疗前升高,观察组高于对照组(P<0.05)。观察组不良反应发生率为6.45%,低于对照组的22.58%,但差异不显著(P>0.05)。结论龙骨小麦疏肝汤治疗甲亢可改善患者临床症状,恢复甲状腺功能,减少复发,提高临床疗效,且安全性较好。

Objective To investigate the effect of Longgu Xiaomai Shugan decoction on recurrence,FT3 and FT4 in patients with liver depression and spleen deficiency hyperthyroidism.Methods Sixty-two patients with hyperthyroidism admitted to our hospital from January 2016 to July 2018 were included.They were divided into the control group(n=31)and the observation group(n=31)according to the random number table method.The control group was treated with methimidazole tablets,and the observation group was treated with Longgu Xiaomai Shugan decoction.Both groups were treated continuously for 3 months and followed up for 1 year.The clinical efficacy of 2 groups after treatment was analyzed.The differences in TCM syndrome scores and thyroid hormone levels between the two groups before and after treatment were compared,and the occurrence of adverse reactions during treatment and recurrence during follow-up were compared between the two groups.Results The total effective rate in the observation group was 96.77%which was higher than 74.19%in the control group(P<0.05),while the recurrence rate in the observation group was 7.14%lower than that in the control group 33.33%(P>0.05).TCM symptom scores and serum FT3,FT4,TT3,and TT3 levels in patients with palpitations,finger tremor,fatigue,irritability,and irritability were lower in the two groups than before treatment.The observation group was lower than the control group(P<0.05).The level was higher than before treatment,and the observation group was higher than the control group(P<0.05).The incidence of adverse reactions in the observation group was 6.45%lower than that in the control group 22.58%,but the difference was not significant(P>0.05).Conclusion Longgu Xiaomai Shugan decoction in the treatment of hyperthyroidism can obviously improve the clinical efficacy,improve the clinical symptoms of the patients,reduce the recurrence,improve the clinical efficacy,and has good safety.
