磷酸铝凝胶联合兰索拉唑治疗幽门螺杆菌阳性胃溃疡疗效分析

Efficacy analysis of aluminum phosphate gel combined with lansoprazole in the treatment of Helicobacter pylori positive gastric ulcer

目的观察磷酸铝凝胶联合兰索拉唑治疗幽门螺杆菌(Hp)阳性胃溃疡的疗效。方法选取余姚市红十字医院2016年1月至2018年8月收治的Hp阳性胃溃疡患者200例,采用随机数字表法分为对照组100例、观察组100例。对照组采取兰索拉唑治疗,观察组采用磷酸铝凝胶联合兰索拉唑治疗,两组疗程均为4周。比较两组临床疗效、Hp根除率、临床症状评分、血清超敏C反应蛋白(hs-CRP)、基质金属蛋白酶9(MMP-9)及胃泌素(GAS)水平,观察两组不良反应发生情况。结果观察组总有效率、Hp根除率分别为96.00%(96/100)、95.00%(95/100),均高于对照组的82.00%(82/100)、86.00%(86/100),差异均有统计学意义(χ^2=10.010、4.711,均P<0.05);观察组反酸嗳气、腹胀、腹痛3项临床症状评分分别为(0.24±0.04)分、(0.13±0.04)分、(0.34±0.05)分,均低于对照组的(0.89±0.08)分、(0.79±0.09)分、(0.96±0.08)分,均差异有统计学意义(t=72.672、67.013、65.720,均P<0.05)。治疗后,观察组hs-CRP、MMP-9、GAS分别为(2.10±0.33)mg/L、(77.96±14.23)μg/L、(123.96±15.40)μg/L,均明显低于对照组的(3.81±0.38)mg/L、(97.20±14.71)μg/L、(168.33±15.27)μg/L,均差异有统计学意义(t=33.976、9.401、20.459,均P<0.05)。两组不良反应发生率差异无统计学意义(P>0.05)。结论磷酸铝凝胶联合兰索拉唑治疗Hp阳性胃溃疡安全有效,可提高Hp根除率,降低血清hs-CRP、MMP-9、GAS水平。

Objective To study the effect of aluminum phosphate gel combined with lansoprazole on Helicobacter pylori(Hp)positive gastric ulcer.Methods From January 2016 to August 2018,200 patients with Hp positive gastric ulcer admitted to Yuyao Red Cross Hospital were selected and divided into two groups by random number table method,with 100 cases in each group.The control group was treated with lansoprazole,while the observation group was treated with aluminum phosphate gel combined with lansoprazole.The course of treatment in both two groups was 4 weeks.The clinical efficacy,Hp eradication rate,clinical symptom score,serum hypersensitive C-reactive protein(hs-CRP),matrix metalloproteinase 9(MMP-9)and gastrin(GAS)levels were compared between the two groups.The occurrence of adverse reactions in the two groups was observed.Results The total effective rate and Hp eradication rate in the observation group were 96.00%(96/100)and 95.00%(95/100),respectively,which were higher than those in the control group[82.00%(82/100)and 86.00%(86/100)],the differences were statistically significant(χ^2=10.010,4.711,all P<0.05).The three clinical symptom scores of the observation group were(0.24±0.04)points,(0.13±0.04)points and(0.34±0.05)points,respectively,which were lower than those of the control group[(0.89±0.08)points,(0.79±0.09)points and(0.96±0.08)points],the differences were statistically significant(t=72.672,67.013 and 65.720,all P<0.05).After treatment,the levels of hs-CRP,MMP-9 and GAS in the observation group were(2.10±0.33)mg/L,(77.96±14.23)g/L and(123.96±15.40)μg/L,respectively,which were significantly lower than those in the control group[(3.81±0.38)mg/L,(97.20±14.71)μg/L and(168.33±15.27)μg/L],the differences were statistically significant(t=33.976,9.401,20.459,all P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Aluminum phosphate gel combined with lansoprazole is safe and effective in treating Hp-positive gastric ulcer,which can improve Hp eradication rate and reduce serum hs-CRP,MMP-9 and GAS levels.
