长期带胰岛素泵与不带泵2型糖尿病患者抑郁与生活质量比较

Comparison of depression and quality of life in patients with type 2 diabetes mellitus with and without insulin pump for a long time

目的:调查长期带胰岛素泵2型糖尿病(T2DM)患者抑郁情况及生活质量,并与不带泵患者进行比较。方法:采用便利抽样法,选择2017年12月—2019年6月某三甲医院内分泌科就诊的长期戴胰岛素泵T2DM患者140例为带泵组,同期不带泵接受胰岛素强化治疗的T2DM患者160例为对照组,应用SDS和自行设计的糖尿病患者生活质量量表进行调查,比较两组抑郁状况与生活质量。结果:带泵组患者抑郁患病率为28.57%,对照组为47.50%,两组比较差异有统计学意义(χ^2=11.281,P<0.01)。带泵组生活质量加权总分值(6.02±1.86)分,优于对照组,差异有统计学意义(t=-5.941,P<0.01)。结论:长期带胰岛素泵的T2DM患者的抑郁患病率较低,生活质量较高,胰岛素泵在改善患者心理状态和提高整体生活质量方面具有积极作用。

Objective To investigate the long-term depression and quality of life in type 2 diabetes mellitus(T2DM)patients with insulin pump for a long time,and compare them with patients without insulin pump.Methods Totally 140 T2DM patients with insulin pump for a long time treated in the Department of Endocrinology at a ClassⅢGrade A hospital from December 2017 to June 2019 were selected by convenient sampling as the pump group,while 160 T2DM patients who underwent intensive insulin therapy without insulin pump were included into the control group.The Self-Depression Scale and the self-designed Quality of Life for Diabetics were used to investigate and compare the depression status and quality of life of the two groups.Results The prevalence of depression in patients in the pump group was 28.57%and that in the control group was 47.50%,and the difference between the two groups was statistically significant(χ^2=11.281;P<0.01).The weighted total score for the quality of life of the pump group was(6.02±1.86),which was better than that of the control group,and the difference was statistically significant(t=-5.941;P<0.01).Conclusions T2DM patients with insulin pump for a long time have a lower prevalence of depression and a higher quality of life.Insulin pumps have a positive role in improving the psychological state of patients and improving the overall quality of life.
