甘露消毒丹加味联合除湿祛疹方治疗小儿手足口病肺脾湿热证50例临床观察

Clinical observation on 50 cases of hand-foot-mouth disease in children with damp-heat of both lung and spleen syndrome treated by modified Ganlu Xiaodu Dan(甘露消毒丹)combined with Chushi Quzhen Fang(除湿祛疹方)

目的观察甘露消毒丹加味联合除湿祛疹方治疗小儿手足口病肺脾湿热证的临床疗效。方法选取2015年3月至2017年10月在北京市昌平区中医医院儿科诊治的手足口病肺脾湿热证患儿100例,按随机数字表法分为治疗组和对照组,各50例。2组均给予常规对症治疗,对照组在此治疗方法的基础上给予抗病毒和抗感染治疗,治疗组在此治疗方法的基础上给予甘露消毒丹加味内服联合除湿祛疹方外洗治疗,5 d为1个疗程,1个疗程后比较临床疗效和治疗前后口腔黏膜、手、足、臀部疱疹,发热,咳嗽,流涕症状积分。结果治疗组总有效率为96.0%(48/50),对照组为78.0%(39/50),2组比较,差异有统计学意义(P<0.05)。治疗前,2组口腔黏膜、手、足、臀部疱疹,发热,咳嗽,流涕症状积分比较,差异均无统计学意义(P>0.05),具有可比性;治疗后,2组的上述症状积分均明显减少,与同组治疗前比较,差异有统计学意义(P<0.05),且治疗组减少更显著(P<0.05)。结论甘露消毒丹加味内服联合除湿祛疹方外洗治疗小儿手足口病肺脾湿热证疗效满意,可有效改善患儿临床症状,值得临床推广应用。

Objective To observe the clinical efficacy of modified Ganlu Xiaodu Dan(甘露消毒丹)combined with Chushi Quzhen Fang(除湿祛疹方)in the treatment of hand-foot-mouth disease in children with damp-heat of both lung and spleen syndrome.Methods From March 2015 to October 2017,100 cases of hand-foot-mouth disease in children with damp-heat of both lung and spleen syndrome diagnosed and treated in pediatrics department of Beijing Changping District Hospital of TCM were divided into treatment group and control group according to random number table method,with 50 cases in each.The 2 groups were both given routine symptomatic treatment,on this basis,the control group was given antiviral and anti-infectious treatment,while the treatment group was given oral administration of modified Ganlu Xiaodu Dan combined with Chushi Quzhen Fang.Five days constituted 1 course of treatment.After 1 course of treatment,the clinical efficacy,the symptom scores of herpes of the mucous membrane of mouth,hand,foot and buttock,fever,cough and running nose before and after treatment were compared.Results The total effective rate of the treatment group was 96.0%(48/50),that of the control group was 78.0%(39/50),and the difference was statistically significant between the 2 groups(P<0.05).Before treatment,the difference was not statistically significant in symptom scores of herpes of the mucous membrane of mouth,hand,foot and buttock,fever,cough and running nose between the 2 groups(P>0.05),and they were comparable.After treatment,the above-mentioned symptom scores of the 2 groups all significantly decreased,and the difference was statistically significant compared with the same group before treatment(P<0.05),moreover,the treatment group decreased more significantly(P<0.05).Conclusion It has satisfactory efficacy and can effectively improve the clinical symptoms of the children patients to use oral administration of modified Ganlu Xiaodu Dan combined with external lavation of Chushi Quzhen Fang in the treatment of hand-foot-mouth disease in children with damp-heat of both lung and spleen syndrome.
