经皮穿刺微球囊压迫术治疗原发性三叉神经痛临床观察

Clinical observation of alteplase intravenous treatment of acute ischemic stroke

目的比较经皮穿刺微球囊压迫术(PMC)与伽玛刀术(GKS)治疗原发性三叉神经痛的临床疗效。方法选取96例原发性三叉神经痛患者为研究对象,其中经皮穿刺微球囊压迫术组51例,伽玛刀术组45例。对比2组术后24 h、6个月、12个月、24个月治愈率及总有效率、并发症发生率以及术后疼痛评分。结果PMC组术后24 h、6个月、12个月、24个月治愈率(94.18%、90.20%、90.20%、88.24%)均显著高于GKS组(13.33%、62.22%、66.67%、68.89%),差异有统计学意义(P<0.05);PMC组术后24 h、6个月有效率(98.04%、98.04%)均显著高于GKS组(68.89%、84.44%),差异有统计学意义(P<0.05);2组术后12个月、24个月总有率比较差异无统计学意义(P>0.05);2组术后并发症发生率比较,差异无统计学意义(P>0.05);2组术后疼痛评分比较,差异有统计学意义(P<0.05)。结论经皮穿刺微球囊压迫术比伽马刀术起效快、操作简单,更适用于疼痛剧烈难以忍受的老年患者。

Objective To compare the clinical efficacy of percutaneous microballoon compression(PMC)and gamma knife(GKS)in the treatment of primary trigeminal neuralgia.Methods Ninety-six patients with primary trigeminal neuralgia were selected as the research objects,including 51 cases in the percutaneous microballoon compression group and 45 cases in the gamma knife group.The cure rate and total effective rate,complication rate and postoperative pain score were compared between the two groups at 24 h,6 months,12 months,and 24 months after operation.Results The cure rates(94.18%,90.20%,90.20%,88.24%)of the PMC group were significantly higher than those of the GKS group(13.33%,62.22%,66.67%,24 hours,6 months,12 months,and 24 months).68.89%),the difference was statistically significant(P<0.05);the effective rates(98.04%,98.04%)of the PMC group at 24 h and 6 months after the operation were significantly higher than those of the GKS group(68.89%,84.44%),the difference was significant statistically significant(P<0.05);there was no significant difference in the total rate of the two groups at 12 months and 24 months after surgery(P>0.05);the difference in the incidence of postoperative complications between the two groups was not statistically significant(P>0.05).The comparison of postoperative pain scores between the two groups was statistically significant(P<0.05).Conclusion Percutaneous puncture microballoon compression has a faster onset and simple operation than gamma knife surgery,and is more suitable for elderly patients with severe and unbearable pain.
