冠状动脉CT血管造影与颈动脉超声在斑块诊断上的相关性分析 被引量:8

摘要 目的:观察颈动脉超声与冠状动脉CT血管造影(CTA)在健康体检者中斑块诊断的相关性。方法:选择在医院体检中心进行体检的150名体检者,根据检查结果分为冠状动脉粥样硬化组(70例)和正常组(80名);将70例冠状动脉粥样硬化组依据其斑块类型再分为非钙化斑块组(60个)和混合斑块组(180个)。所有体检者均接受64排螺旋CT检查,同时应用高频超声测量颈总动脉内膜中层厚度(IMT)与斑块形态。结果:冠状动脉粥样硬化组中IMT厚度增加、颈动脉粥样硬化斑块形成者59例(占84.3%);冠状动脉正常组中IMT厚度增加、颈动脉粥样硬化斑块形成者30例(占37.5%),两组比较差异有统计学意义(x^2=33.868,P<0.05);冠状动脉粥样硬化组中的非钙化斑块组颈动脉软斑发生率明显高于混合斑块组,差异有统计学意义(x^2=51.673,P<0.05)。结论:颈动脉超声斑块形态与冠状动脉CTA斑块类型有一定相关性。 Objective: To observe the correlation between carotid artery ultrasound and coronary artery computed tomography angiography(CTA) in healthy persons who underwent physical examination. Methods: 150 persons who underwent physical examination in physical examination center of hospital were selected. And they were divided into coronary atherosclerosis group(70 cases) and normal coronary artery group(80 cases) according to the results of examination. And then coronary atherosclerosis group(70 cases) were further divided into non-calcified plaque group(60 cases) and mixed plaque group(180 cases) according to the type of plaque. All of them received 64-slice spiral CT examination. Meanwhile, high-frequency ultrasound was used to measure the intima-media thickness(IMT) and plaque morphology of the common carotid artery. Results: In coronary atherosclerosis group, the number of patients with increased IMT thickness and carotid atherosclerosis plaque formation was 59 cases(84.3%). In normal coronary artery group, the number of those was 30 cases(37.5%). The difference of the incidence between the two groups was significant(x^2=33.868, P<0.05). In coronary atherosclerosis group, the incidence rate of soft plaque of carotid artery of noncalcified plaque group was significantly higher than that of mixed plaque group(x^2=51.673, P<0.05). Conclusion: There is a certain correlation between plaque morphology of carotid artery ultrasound and the type of plaque of coronary artery CTA.
