引入MDT理念强化不可收费耗材成本管理

Introducing the MDT Concept to Strengthen the Cost Management of Non-Chargeable Consumables

目的通过分析本院介入不可收费耗材的管理现状,探索耗材多学科协作(Multi-Disciplinary Team,MDT)管理团队,在介入手术室不可收费耗材管理中的应用成效。方法借鉴临床MDT诊疗模式,组成分管院领导、医用耗材管理中心、信息处、物价处、介入手术室耗材MDT管理团队,建立完善制度及管理体系、实行重点流程质量控制管理、借助信息化系统平台管理、强化物价管理、运用管理工具进行持续质量改进五大管理对策。结果介入手术室不可收费耗材成本控制成效明显,2017年同比2016年收入增长7.26%,工作量同比增长6.43%,而不可收费耗材金额同比下降31.90%;2018年同比2017年收入增长17.58%,工作量同比增长25.30%,而不可收费耗材金额同比增长却只有1.59%;进一步规范了不可收费耗材管理架构和制度流程;形成成本管理长效机制。结论通过建立多部门耗材MDT管理团队,在保证医疗安全的基础上,有效控制不可收费耗材费用支出,利于降低医院运营成本、节能降耗、提升内涵质量及社会竞争力,对引领医改付费制度改革、疾病诊断相关分组等起到促进作用。

Objective To explore the application effect of multi-disciplinary team(MDT)in management of non-chargeable consumables in interventional operating rooms by analyzing the current situation of management of non-chargeable consumables in our institute.Methods By referring to the clinical MDT diagnosis and treatment model,the management team of the hospital in charge,the medical consumables management center,the information department,the price department and the MDT management team of consumables in the interventional operating room were formed.Five management countermeasures that including establishing and improving the system and management system,implementing the quality control management of key process,managing with the help of information system platform,strengthening the price management and using management tools for continuous quality improvement were performed.Results The cost control effect of non-chargeable consumables in intervention-operating room was obvious.In 2017,the annual revenue increased by 7.26%,the workload increased by 6.43%,and the amount of non-chargeable consumables decreased by 31.90%.In 2018,the annual revenue increased by 17.58%and the workload increased by 25.30%,while the amount of non-chargeable consumables increased by only 1.59%.Further standardizing the management framework and system process of non-chargeable consumables,and form a long-term cost management mechanism.Conclusion Through the establishment of multi-department consumable MDT management team,on the basis of ensuring medical security,the cost of non-chargeable consumables can be effectively controlled,which is conducive to the reduction of hospital operating costs,energy conservation and consumption reduction,the improvement of connotation and quality and social competitiveness,and plays a promoting role in leading the reform of medical reform payment system and the grouping of disease diagnosis.
