糖尿病人群自然步态下足底压力分布研究 被引量:7

摘要 为研究糖尿病和足病患者足底压力变化的特征和差异,采用鞋垫式足底压力检测系统测量了自然步态下糖尿病非足病患者(DM,n=83)和糖尿病足病患者(DF,n=32)的足底压力.分析发现,与健康对照组(NC,n=55)相比,DM、DF组的冲量百分比和峰值压力主要在第二至第五跖骨区(M2—M5)有所升高,多数足区增幅分别在50%~80%和60%~100%之间,明显向前足跖骨区转移;DF组在足跟中部(HC)、足跟外侧(HL)的冲量百分比降幅为15%~30%.与DM组相比,DF组冲量百分比和峰值压力在中足区(MF)均有所升高.对于包络压力中心点95%椭圆置信面积,NC组>DM组>DF组.研究糖尿病人群足底压力特征,有助于加强对糖尿病足的临床预防和鞋垫设计. To explore the characteristics and differences of foot pressure changes in patients with diabetes and podiatry,the insole type of foot pressure detection system was used to measure the foot pressure of diabetic patients without foot disease(DM,n=83)and with foot disease(DF,n=32)in natural gait.Analysis found that compared with the healthy control group(NC,n=55),the percentage of impulse and peak pressure of both the DM and DF groups were increased mainly in the second to fifth metatarsal areas(M2-M5),and the increase in most foot areas was 50%-80%and 60%-100%.Plantar pressure was clearly transferred to the tibia.The percentage of impulse in the middle of the heel(HC)and the lateral part of the heel(HL)of the DF group decreased by 15%-30%.Compared with the DM group,the impulse percentage and peak pressure of the DF group both increased in the MF region.As to 95%elliptical confidence area of the envelope pressure center point,the pressure was NC group>DM group>DF group.Studying the characteristics of plantar pressure in diabetic patients will help strengthen the clinical prevention and insole design of diabetic foot.
