桃仁承气汤联合西医序贯治疗对新诊2型糖尿病患者短期胰岛素泵强化治疗的影响

Influence of Taoren Chengqi Decoction combined with Western medicine sequential therapy on short-term insulin pump intensive treatment of newly diagnosed type 2 diabetes

目的分析桃仁承气汤联合西医序贯治疗对新诊2型糖尿病患者短期胰岛素泵强化治疗的影响。方法选取2019年11月~2020年1月湖南中医药高等专科学校附属第一医院收治的已接受3周胰岛素泵强化治疗的120例新诊2型糖尿病患者作为研究对象,通过随机数字表法将其分成A、B、C、D四组,每组各30例,分别给予艾塞那肽、桃仁承气汤联合艾塞那肽、利拉鲁肽、桃仁承气汤联合利拉鲁肽治疗。观察比较四组患者的空腹血糖(FPG)、胰岛素抵抗指数(HOMA-IR)、胰岛素曲线下面积(AUC_(Gln0~3 min))、胰岛α细胞功能(AUC_(Ins0~10 min))、胰岛β细胞功能指数(HOMA-β)、体重指数(BMI)。结果治疗后,四组患者的FPG、HOMA-IR、AUC_(Gln0~3 min)均低于治疗前,AUC_(Ins0~10 min)以及HOMA-β均高于治疗前,差异有统计学意义(P<0.05);治疗前后四组患者的BMI比较,差异无统计学意义(P>0.05)。治疗后,A组与C组患者的FPG、HOMA-IR、AUC_(Gln0~3 min)、AUC_(Ins0~10 min)以及HOMA-β比较,差异无统计学意义(P>0.05),B组与D组患者的FPG、HOMA-IR、AUC_(Gln0~3 min)、AUC_(Ins0~10 min)以及HOMA-β比较,差异无统计学意义(P>0.05);治疗后,B组与D组患者的FPG、HOMA-IR、AUC_(Gln0~3 min)、AUC_(Ins0~10 min)以及HOMA-β均优于A组,差异有统计学意义(P<0.05);治疗后,D组患者的FPG、HOMA-IR、AUC_(Gln0~3 min)、AUC_(Ins0~10 min)以及HOMA-β均优于C组,差异有统计学意义(P<0.05);治疗后,B组与C组患者的各项临床指标比较,差异有统计学意义(P<0.05);治疗后四组患者的BMI比较,差异无统计学意义(P>0.05)。结论初诊2型糖尿病患者接受桃仁承气汤联合西医序贯治疗,能对其血糖水平进行有效控制,使血糖水平保持稳定,具有临床推广价值。

Objective To analyze the influence of Taoren Chengqi Decoction combined with Western medicine sequential therapy on short-term insulin pump intensive treatment of newly diagnosed type 2 diabetes.Methods A total of 120 newly diagnosed type 2 diabetes patients who had received insulin pump intensive treatment for 3 weeks in the First Affiliated Hospital of Hu′nan Traditional Chinese Medical College from November 2019 to January 2020 were selected as the research objects.They were divide into group A,B,C,D according to the random number table method,30 cases in each group,and patients in four groups were treated with Exenatide,Taoren Chengqi Decoction combined with Exenatide,Liraglutide,Taoren Chengqi Decoction combined with Liraglutide respectively.The fasting plasma glucose(FPG),insulin resistance index(HOMA-IR),area under the insulin curve(AUCGln0-3 min),isletαcell function(AUCIns0-10 min),isletβcell function index(HOMA-β),body mass index(BMI)were observed and compared.Results After treatment,the FPG,HOMA-IR,AUCGln0-3 min of the four groups were lower than those before treatment,AUCIns0-10 min and HOMA-βwere higher than those before treatment,the differences were statistically significant(P<0.05).There was no statistically significant difference in BMI of the four groups between before and after treatment(P>0.05).After treatment,there were no significant differences in FPG,HOMA-IR,AUCGln0-3 min,AUCIns0-10 min and HOMA-βbetween group A and group C(P>0.05).There were no significant differences in FPG,HOMA-IR,AUCGln0-3 min,AUCIns0-10 min and HOMA-βbetween group B and group D(P>0.05).After treatment,the FPG,HOMA-IR,AUCGln0-3 min,AUCIns0-10 min and HOMA-βof the patients in group B and group D were better than those in group A,and the differences were statistically significant(P<0.05).After treatment,the FPG,HOMA-IR,AUCGln0-3 min,AUCIns0-10 min and HOMA-βof group D were better than those of group C,and the differences were statistically significant(P<0.05).There were statistically significant differences in the clinical indicators of patients between group B and group C after treatment(P<0.05).There was no statistically significant difference in the BMI of the four groups after treatment(P>0.05).Conclusion Taoren Chengqi Decoction combined with Western medicine sequential therapy can effectively control the blood glucose levels of newly diagnosed type 2 diabetic patients,keep the blood glucose level stable,which has clinical promotion value.
