七氟烷复合右美托咪定在小儿唇腭裂手术中的应用

Application of Sevoflurane combined with Dexmedetomidine in pediatric cheilopalatognathus surgery

目的探讨七氟烷复合右美托咪定在小儿唇腭裂手术中的应用。方法选取2018年1月~2019年9月锦州医科大学附属第二医院收治的50例唇腭裂患儿作为研究对象,按照随机数字表法分为C组和D组,每组各25例。C组采用单纯七氟烷吸入麻醉法,D组采用七氟烷复合右美托咪定麻醉法。比较两组麻醉开始时(T0)、手术开始时(T_1)、拔管时(T_2)的心率(HR)、平均动脉压(MAP);比较两组患者的苏醒时间及不良反应发生情况。结果D组T_1、T_2时的HR、MAP低于C组,差异有统计学意义(P<0.05)。C组T_1、T_2时HR、MAP高于T_0,差异有统计学意义(P<0.05)。D组T_0、T_1、T_2时的HR及MAP比较,差异无统计学意义(P>0.05)。两组患者的苏醒时间比较,差异无统计学意义(P>0.05)。D组患者苏醒期躁动发生率低于C组,差异有统计学意义(P<0.05)。结论七氟烷复合右美托咪定应用于小儿唇腭裂手术中,血流动力学平稳,苏醒期躁动发生率低。

Objective To investigate the application of Sevoflurane combined with Dexmedetomidine in pediatric cheilopalatognathus surgery.Methods A total of 50 children patients with cheilopalatognathus who received treatment between January 2018 and September 2019 in the Second Affiliated Hospital of Jinzhou Medical University were selected as the research objects.The selected children were divided into group C and group D according to random number table method,25 cases in each group.Group C used Sevoflurane alone as inhalation anesthesia,and the group D used Sevoflurane combined with Dexmedetomidine.The heart rate(HR),mean arterial pressure(MAP)were compared between the two groups at the beginning of anesthesia(T0),at the beginning of operation(T1)and at the time of extubation(T2).The recovery time and the occurrence of adverse reactions were compared between the two groups.Results The HR and MAP at T1 and T2 in group D were lower than those in group C,the differences were statistically significant(P<0.05).In group C,HR and MAP were higher at T1 and T2 than those at T0,the differences were statistically significant(P<0.05).In group D,there were no significant differences in HR and MAP at T0,T1 and T2(P>0.05).There was no significant difference in the recovery time between the two groups(P>0.05).The incidence of emergence agitation in group D was lower than that in group C,the difference was statistically significant(P<0.05).Conclusion Sevoflurane combined with Dexmedetomidine in pediatric cheilopalatognathus surgery achieves stable hemodynamics and low incidence of emergence agitation.
