彩色多普勒超声联合心电图检查在高血压心脏病诊断中的价值

Value of color Doppler ultrasound combined with electrocardiogram in the diagnosis of hypertensive heart disease

目的探讨彩色多普勒超声联合心电图检查在高血压心脏病中的诊断价值。方法选取2019年1~12月我院的72例疑似高血压心脏病患者,患者入院后均行心电图检查和彩色多普勒超声检查。比较心电图、彩色多普勒超声以及两者联合检查诊断高血压心脏病的敏感度、特异度及准确率,并比较心电图、彩色多普勒超声以及联合检查对不同高血压心脏病类型的检出率。结果彩色多普勒超声检查、联合检查诊断高血压心脏病的敏感度、特异度及准确率均高于心电图检查,差异有统计学意义(P<0.05)。彩色多普勒超声检查与联合检查的敏感度、特异度及准确率比较,差异无统计学意义(P>0.05)。彩色多普勒超声检查及联合检查对左心室扩大、左心室后壁肥厚、室间隔肥厚、左心室肥厚的检出率显著高于心电图检查,差异有统计学意义(P<0.05)。彩色多普勒超声检查的左心室扩大检出率低于联合检查(P<0.05);彩色多普勒超声检查与联合检查对左心室后壁肥厚、室间隔肥厚、左心室肥厚的检出率比较,差异无统计学意义(P>0.05)。结论彩色多普勒超声检查在高血压心脏病中的应用价值显著,诊断效能高于心电图,同时联合心电图检查有利于提高高血压心脏病患者检查的准确率。

Objective To explore the value of color Doppler ultrasound combined with electrocardiogram in the diagnosis of hypertensive heart disease.Methods From January 2019 to December 2019,72 patients suspected with hypertensive heart disease in our hospital were selected.After admission,all patients were examined by electrocardiogram and color Doppler ultrasound.The sensitivity,specificity and accuracy of electrocardiogram,color Doppler ultrasound and their combined examination in the diagnosis of hypertensive heart disease were compared,and the detection rates of electrocardiogram,color Doppler ultrasound and their combined examination in different types of hypertensive heart disease were compared.Results The sensitivity,specificity and accuracy of color Doppler ultrasound and combined examination in the diagnosis of hypertensive heart disease were higher than those of electrocardiogram,the differences were statistically significant(P<0.05).There were no significant differences in the sensitivity,specificity and accuracy between color Doppler ultrasound and combined examination(P>0.05).The detection rates of left ventricular enlargement,left ventricular posterior wall hypertrophy,interventricular hypertrophy and left ventricular hypertrophy of color Doppler ultrasound and combined examination were significantly higher than those of electrocardiogram,the differences were statistically significant(P<0.05).The detection rate of left ventricular enlargement of color Doppler ultrasound was lower than thoat of combined examination(P<0.05),and there were no significant difference in the detection rates of left ventricular posterior wall hypertrophy,interventricular hypertrophy and left ventricular hypertrophy between color Doppler ultrasound and combined examination(P>0.05).Conclusion The application value of color Doppler ultrasound in hypertensive heart disease is significant,and the diagnostic efficiency is higher than that of electrocardiogram,which combined with electrocardiogram examination is helpful to improve the accuracy of the examination of patients with hypertensive heart disease.
