玻璃酸钠滴眼液联合重组人表皮生长因子滴眼液改善老年白内障患者术后角膜损伤和修复角膜内皮细胞的研究

Study of effect of sodiumhyaluronate eye drop combinedwith recombinant human epidermal growth factor eye drop on corneal injure and corneal endothelial cell repair in elderly patients after cataract surgery

目的观察玻璃酸钠滴眼液联合重组人表皮生长因子(rh-Epidermal growth factor,rhEGF)滴眼液改善老年白内障患者的术后角膜损伤及对角膜内皮细胞修复的影响。方法选取2018年7月—2020年2月于重庆市第五人民医院收治的老年白内障术后患者100例,随机分为观察组(n=50)和对照组(n=50)。观察组采取玻璃酸钠滴眼液联合rhEGF滴眼液治疗,对照组采用玻璃酸钠滴眼液治疗。评估并比较分析2组患者的治疗效果、角膜内皮细胞密度、六角形细胞比例和角膜内皮细胞变异系数和炎症反应。结果观察组治疗总有效率明显高于对照组(96.00%vs 78.00%,P<0.05)。治疗后,2组角膜内皮细胞密度和六角形细胞比例均明显高于同组治疗前,角膜内皮细胞变异系数均明显低于同组治疗前(P<0.05);观察组角膜内皮细胞密度和六角形细胞比例均明显高于对照组,而角膜内皮细胞变异系数明显低于对照组(P<0.05)。治疗后,2组泪液中IL-6、TNF-α和IL-1水平均明显低于同组治疗前,而观察组均明显低于对照组(P<0.05),提示观察组炎症反应比对照组改善更明显。结论玻璃酸钠滴眼液联合rhEGF滴眼液对老年白内障术后患者的角膜损伤具有较好的改善作用,可能与其修复角膜细胞损伤和减轻炎症反应相关,具有一定的临床应用价值。

Objective To explore the effect of sodiumhyaluronate eye drop combinedwith recombinant human epidermal growth factor(rhEGF)eye drop on corneal injure and corneal endothelial cell repair in elderly patients after cataract surgery.Methods From July 2018 to February 2020,100 elderly patients after cataract surgery in fifth people's hospital of Chongqing city were selected and randomly divided into observation group(n=50)and control group(n=50).The observation groupwas treated with sodium hyaluronate eye drop combined with rhEGF eye drop,while the control group was treated with sodium hyaluronate eye drop.The therapeutic effect,corneal endothelial cell density,percentage of hexagonal cells,coefficient of variation of corneal endothelial cells and inflammatory reaction were evaluated and compared between the two groups.Results The total effective rate of the observation groupwas significantly higher than that of the control group(96.00%vs 78.00%,P<0.05).After treatment,the density of corneal endothelial cells and the proportion of hexagonal cells in both groups were significantly higher than those before treatment in the same group,and the coefficient of variation of corneal endothelial cells was significantly lower than that before treatment in the same group(P<0.05).After treatment,the levels of IL-6,TNF-α and IL-1 in tears of the two groups were significantly lower than those before treatment in the same group,while those in the observation group were significantly lower than those in the control group(P<0.05),which suggested that the inflammatory response in the observation group improved more significantly than in the control group.Conclusion Sodium hyaluronate eye drop combined with rhEGF eye drop can improve corneal injury in elderly patients after cataract surgery,which may be related to the repair of corneal cell damage and the reduction of inflammatory reaction,which has certain clinical application value.
