摘要 目的评价高强度聚焦超声(HIFU)治疗老年胰腺癌患者的有效性,并探讨影响其预后的因素。方法收集2016年2月-2018年10月于本科住院诊治的≥60岁胰腺癌患者的临床资料,对67例不能手术的老年胰腺癌患者行HIFU治疗,采用Kaplan-Meier法行影响预后的单因素分析,Cox回归模型分析可能影响预后的独立危险因素。结果所有患者的疾病控制率达68.7%,中位PFS为6个月。TNM分期早、无肝转移、肝功能正常的患者PFS预后更佳,是影响PFS的独立预后因素。结论HIFU治疗能在一定程度上延长老年胰腺癌患者的生存期。HIFU治疗后的生存期与患者的TMN分期、有无肝转移、肝功能情况有关。 Objective To evaluate the effectiveness of high-intensity focused ultrasound(HIFU)in the treatment of elderly patientswith pancreatic cancer,and to explore the factors affecting their prognosis.Methods The clinical data of pancreatic cancer patients over 60 years old who were hospitalized in our department from February 2016 to October 2018 were collected.Sixty seven elderly patients with unresectable pancreatic cancer were treated with HIFU.Kaplan-Meier method was used for univariate analysis of prognosis,and Cox regression model was used to analyze independent risk factors that may affect prognosis.Result The disease control rate of all patients was 68.7%,and the median PFS was 6 months.Patients with early TNM stage,no liver metastasis and normal liver function had a better prognosis,which were the independent prognostic factors of PFS.Conclusion HIFU can prolong the survival time of elderly patients with pancreatic cancer to a certain extent.The survival time after HIFU treatment is related to TMN stage,liver metastasis and liver function.
