1例VHL综合征伴双肾癌患者行双肾部分切除术后并发急性肾功能衰竭的术后护理

Postoperative nursing of a patient with VHL syndrome and bilateral renal carcinoma complicated with acute renal failure after bilateral partial nephrectomy

目的探讨1例Von Hippel-Lindau(VHL)综合征伴双肾癌患者行双肾部分切除术后并发急性肾功能衰竭的术后护理。方法以本院收治的1例VHL综合征患者为研究对象,行经腹双肾部分切除术后并发急性肾功能衰竭,接受连续肾脏替代疗法(CRRT)治疗。术后给予针对性的护理,动态观察,预见性地预防及处置并发症,重视心理护理。结果经过短期的CRRT治疗,患者肾功能恢复,3周后康复出院。出院后定期随诊,尿量及肾功能基本正常。结论临床上对于VHL综合征伴双肾癌的患者常规采用先后双肾切除或部分切除术,本案例患者同时行双肾部分切除术,避免了再次手术的风险,经过及时精心的治疗和护理,取得了满意的效果。

Objective To investigate the postoperative nursing of a patient with Von Hippel-Lindau(VHL)syndrome and bilateral renal carcinoma complicated with acute renal failure after bilateral partial nephrectomy.Methods A VHL syndrome patient admitted in our hospital was taken as the study object.The patient complicated with acute renal failure after transabdominal bilateral partial nephrectomy,received continuous renal replacement therapy(CRRT)treatment.After operation,the patient was given targeted nursing,dynamic observation,predictively give prevention and treatment of complications,and psychological nursing was emphasized.Results After short-term CRRT treatment,the renal function of the patient recovered and discharged 3 weeks later.Regular follow-up after discharge showed that urine output and renal function were basically normal.Conclusion Patients with VHL syndrome and bilateral renal carcinoma were routinely treated with double nephrectomy or partial resection successively in clinic.In this case,patient was treated with bilateral partial nephrectomy simultaneously to avoid the risk of reoperation.After timely and careful treatment and nursing,satisfactory results were achieved.
