经电子支气管镜采样的支气管肺泡灌洗液对下呼吸道感染的诊断价值 被引量:7

Diagnostic value of bronchoalveolar lavage fluid sampled by electronic bronchoscope for lower respiratory tract infection

摘要 目的探讨经电子支气管镜采样的支气管肺泡灌洗液对下呼吸道感染的诊断价值。方法采集264例疑似下呼吸道感染患者的支气管肺泡灌洗液及痰液进行病原菌培养,根据送检标本采样方式的不同分为对照组与研究组。对照组采用常规方式取痰,研究组则经电子支气管镜采集支气管肺泡灌洗液。比较两组的标本合格率、合格标本病原菌检出率及诊断符合率。结果研究组的标本合格率及合格标本病原菌检出率均高于对照组(P<0.05)。研究组阳性符合率及阴性符合率均高于对照组(P<0.05)。结论经电子支气管镜采集的支气管肺泡灌洗液可有效避免口咽部定植菌污染,其标本合格率、合格标本病原菌检出率及诊断符合率均较高,对下呼吸道感染诊断有重要临床意义。 Objective To explore the diagnostic value of bronchoalveolar lavage fluid sampled by electronic bronchoscope for lower respiratory tract infection.Methods The bronchoalveolar lavage fluid and sputum of 264 patients with suspected lower respiratory tract infection were collected for pathogen culture.The samples were divided into control group and study group according to different sampling methods.In the control group,sputum was collected by conventional way,while bronchoalveolar lavage fluid was collected by electronic bronchoscope in the study group.The qualified rate of samples,the detection rate of pathogenic bacteria in qualified samples and diagnostic coincidence rate of the two groups were compared.Results The qualified rate and detection rate of pathogenic bacteria in qualified samples in the study group were higher than those in the control group(P<0.05).The positive coincidence rate and negative coincidence rate in the study group were higher than those in the control group(P<0.05).Conclusion Bronchoalveolar lavage fluid collected by electronic bronchoscope can effectively avoid contamination by oral and pharyngeal colonization bacteria,it has high qualified rate of samples,the detection rate of pathogenic bacteria in qualified samples and diagnostic coincidence rate,which has important clinical significance for the diagnosis of lower respiratory tract infection.
