呼吸反馈训练联合振动排痰机排痰对脑卒中气管切开患者肺部感染和生活质量的影响

Effect of respiratory feedback training combined with sputum excretion by vibrating sputum excretion machine on pulmonary infection and quality of life in stroke patients with tracheotomy

目的探究呼吸反馈训练联合振动排痰机排痰对脑卒中气管切开患者肺部感染和生活质量的影响。方法选取我院收治的66例脑卒中气管切开的患者,采用随机数字表法将其分为对照组(33例,呼吸反馈训练+人工排痰)和研究组(33例,呼吸反馈训练+振动排痰机排痰)。比较两组患者的干预效果。结果干预后,两组的日排痰量均增加,最大吸气压和最大呼气压均增大,生活质量评分均下降,且研究组明显优于对照组(P<0.05)。研究组的气管拔管率高于对照组,肺部感染率低于对照组(P<0.05)。结论呼吸反馈训练联合振动排痰机可通过提高脑卒中气管切开患者呼吸肌力,促进排痰,预防和减少肺部感染,改善患者的生活质量。

Objective To explore the effect of respiratory feedback training combined with sputum excretion by vibrating sputum excretion machine on pulmonary infection and quality of life in stroke patients with tracheotomy.Methods A total of 66 stroke patients with tracheotomy in our hospital were selected and randomly divided into control group(33 cases,respiratory feedback training+artificial expectoration)and study group(33 cases,respiratory feedback training+sputum excretion by vibrating sputum excretion machine)according to the random number table method.The intervention effects of the two groups were compared.Results After intervention,the daily sputum output,maximum inspiratory pressure and maximum expiratory pressure in the two groups increased,and the quality of life scores decreased,and those in the study group were significantly better than the control group(P<0.05).The extubation rate in the study group was higher than that in the control group,and the pulmonary infection rate was lower than that in the control group(P<0.05).Conclusion Respiratory feedback training combined with vibrating sputum excretion machine can improve the respiratory muscle strength of stroke patients with tracheotomy,promote expectoration,prevent and reduce pulmonary infection,and improve the quality of life of patients.
