替加环素联合头孢哌酮钠舒巴坦钠治疗血液病患者多重耐药鲍曼不动杆菌感染的效果

Effect of tigecycline combined with cefoperazone sodium and sulbactam sodium in the treatment of multi-drug resistant Acinetobacter baumannii infection in patients with hematological diseases

目的分析替加环素联合头孢哌酮钠舒巴坦钠治疗血液病患者多重耐药鲍曼不动杆菌感染的效果。方法选择我院2017年1月至2019年12月收治的60例血液病多重耐药鲍曼不动杆菌感染患者,将其随机分为对照组和研究组,各30例。对照组采用头孢哌酮钠舒巴坦钠进行治疗,研究组采用替加环素联合头孢哌酮钠舒巴坦钠进行治疗。比较两组的治疗效果。结果研究组的治疗总有效率为83.3%,高于对照组的60.0%(P<0.05)。研究组的细菌有效清除率为66.7%,高于对照组的40.0%(P<0.05)。研究组的体温恢复时间、肺部湿啰音消失时间以及痰菌转阴时间均短于对照组(P<0.05)。治疗后,两组患者白细胞计数、C反应蛋白、降钙素原及肿瘤坏死因子α水平均较治疗前降低,且研究组低于对照组(P<0.05)。结论替加环素联合头孢哌酮钠舒巴坦钠治疗血液病患者多重耐药鲍曼不动杆菌感染的效果确切,可以提高细菌有效清除率,促进临床症状改善,减轻炎症反应。

Objective To analyze the effect of tigecycline combined with cefoperazone sodium and sulbactam sodium in the treatment of multi-drug resistant Acinetobacter baumannii infection in patients with hematological diseases.Methods Sixty patients with hematologic diseases infected by multi-drug resistant Acinetobacter baumannii in our hospital from January 2017 to December 2019 were selected and randomly divided into control group and study group,with 30 cases in each group.The control group was treated with cefoperazone sodium and sulbactam sodium,and the study group was treated with tigecycline combined with cefoperazone sodium and sulbactam sodium.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the study group was 83.3%,which was higher than 60.0%of the control group(P<0.05).The bacterial effective clearance rate of the study group was 66.7%,which was higher than 40.0%of the control group(P<0.05).The time of body temperature recovery,the time of lung moist rales disappearance and the time of sputum bacteria turning negative in the study group were shorter than those in the control group(P<0.05).After treatment,white blood cell count,the levels of C-reactive protein,procalcitonin and tumor necrosis factor-αin the two groups were lower than those before treatment,and those of the study group were lower than the control group(P<0.05).Conclusion Tigecycline combined with cefoperazone sodium and sulbactam sodium is effective in the treatment of multi-drug resistant Acinetobacter baumannii infection in patients with hematological diseases,which can improve the bacterial effective clearance rate,promote the improvement of clinical symptoms and reduce the inflammatory reaction.
