基于倾向值匹配法的湿化与非湿化中低流量鼻导管吸氧的差异性研究

Using propensity score matching to test the difference between humidified and non-humidified low-to-moderate flow oxygen therapy by nasal cannula

目的比较湿化与非湿化中低流量鼻导管氧疗患者氧疗舒适度的差异,为中低流量鼻导管氧疗是否需要湿化提供参考依据。方法采用一般资料调查表和氧疗舒适度问卷对温州医科大学附属第一医院和浙江省金华市中心医院呼吸内科、心血管内科、心胸外科、泌尿外科4个科室的482例经鼻导管进行中低流量氧疗的患者进行调查。采用倾向值匹配方法共匹配出308例患者,分为湿化组与非湿化组各154例,分析两组患者氧疗舒适度的差异。结果湿化组与非湿化组患者的口腔干燥、口腔烧灼感、说话困难、口渴、喉咙干燥、喉咙痛和/或吞咽困难、鼻腔干燥得分比较,差异无统计学意义(P>0.05),两组患者在氧疗舒适度总得分方面,差异无统计学意义(P>0.05)。结论非湿化中低流量鼻导管给氧方式值得推广。

Objective To compare the differences in comfort between humidified and non-humidified low-to-moderate flow oxygen therapy via nasal cannula,and to provide reference for the selection of humidification methods for low-to-moderate flow oxygen therapy.Methods Participants come from four departments,respiratory medicine,cardiovascular medicine,cardiothoracic surgery and urology surgery in two tertiary hospitals in Zhejiang province.The general information questionnaire and oxygen therapy comfort questionnaire were used to investigate 482 patients who were undergoing low-to-moderate flow oxygen therapy via nasal cannula.The propensity score matched(PSM)method was used to obtain 154 paired samples from humidification group and non-humidification group.The difference of oxygen therapy comfort between the two groups was analyzed.Results There was no significant difference in the scores of oral dryness,oral burning sensation,speaking difficulty,thirst,throat dryness,sore throat and/or swallowing difficulty and nasal dryness between the two groups(P<0.05).There was no significant difference in the total score of oxygen therapy comfort between humidification group and non-humidification group(P<0.05).Conclusion The oxygen supply mode of non-humidified low-to-moderate flow oxygen therapy via nasal cannula is worthpopularizing.
