壮方柔肝化纤颗粒联合自体骨髓干细胞治疗乙肝患者肝纤维化、炎症因子及外周血T细胞的影响

Effect of Zhuangfang Rougan Huaxian Granules combined with Autologous Bone marrow stem Cells on liver fibrosis, inflammation factors and T Cells in Peripheral Blood in Hepatitis B Patients

目的探讨壮方柔肝化纤颗粒联合自体骨髓干细胞治疗乙肝患者肝纤维化、炎症因子及外周血中T淋巴细胞的影响及意义。方法采用随机数字表法,将2017年4月~2019年4月确诊为乙肝肝纤维化患者68例,分为治疗组和对照组,每组34例。对照组给予自体骨髓间质干细胞移植,治疗组在对照组的基础上加用柔肝化纤颗粒,疗程为6个月。观察两组的总有效率;中医证候积分;肝纤维化指标、门静脉宽度、脾脏厚度;炎症因子及外周血T细胞的变化。结果治疗后两组相比,治疗组临床总有效率明显高于对照组(P<0.05);治疗组患者肝纤维化指标下降幅度较对照组显著(P<0.05);治疗组患者门静脉宽度、脾脏厚度较对照组下降显著(P<0.05);治疗组炎症因子(TNF-α、IL-6、IL-8)水平均明显低于对照组,IL-10高于对照组(P<0.05);外周血T细胞(CD3+、CD4+、CD4+/CD8+)均升高、CD8+显著下降(P<0.05)。结论壮方柔肝化纤颗粒联合自体骨髓干细胞在治疗乙肝肝纤维化具有显著的临床疗效,包括改善肝纤维化指标、门静脉宽度、脾脏厚度、炎症因子等方面,并且能提高患者机体免疫应答,值得临床借鉴。

Objective To discuss the effect and significance of Zhuangfang Rougan Huaxian Granules combined with Autologous Bone marrow stem cells on liver fibrosis, inflammation factors, T cells in Peripheral Blood in Hepatitis B Patients. Methods Using random number table method, a total of 68 patients from April 2017 to April 2019 who were diagnosed liver fibrosis of hepatitis B divide into two groups, 34 cases each group. The Control group was given Autologous Bone marrow stem cells transplantation, the treatment group added Rougan Huanxian Granules on the base of the control group, the course of treatment was six months. To observe the two group’s change of total efficiency, integral of TCM syndrome, liver fibrosis indicators, width of portal vein, thickness of spleen, inflammation factors, T cells in peripheral Blood. Results Comparing the two groups of after treatment, the treatment group of clinical total efficiency was higher than the control group(P<0.05), the liver fibrosis indicators reduced more evidently than the control group(P<0.05), the width of portal vein and thickness of spleen decreased more obviously than the control group(P<0.05), and the inflammation factors(TNF-α, IL-6, IL-8) level were obviously lower than the control group, while the IL-10 was higher than that(P<0.05). T cells(CD3+, CD4+, CD4+/CD8+) in peripheral Blood all were increased, while the CD8+ was decreased(P<0.05). Conclusion Zhuangfang Rougan Huaxian Granules combined with Autologous Bone marrow stem cells have remarkable clinical effect on liver fibrosis of hepatitis B, including improving the liver fibrosis indicators, width of portal vein, thickness of spleen and inflammation factors. Also it can improve the immune response of patienc ts, it is worthy of clinical reference.
