MRI和螺旋CT在乙肝肝硬化继发肝癌诊断价值中的对比研究

Comparative study of MRI and spiral CT in the diagnosis of secondary hepatocellular carcinoma with hepatitis B cirrhosis

目的:MRI和螺旋CT在乙肝肝硬化继发肝癌诊断价值中的对比研究.方法:选取2017-01~2018-12濮阳市中医医院收治的乙肝肝硬化患者300例,入选患者均采用行腹部螺旋CT与MRI检查.观察螺旋CT与MRI平扫和增强扫描检出肝癌病灶的检查结果,比较两种检查方法在肝癌病灶检测中的结果.结果:经MRI检查共检出病灶67个,平扫T1期、T2期检出率分别为70.15%、73.13%,延迟期、门静脉期、动脉期检出率分别为74.63%、62.69%、88.06%.平扫与增强的检出率相较有统计学差异(P<0.05);经CT检查共检出病灶58个,平扫期检出率为53.45%,延迟期、门静脉期、动脉期检出率分别为70.69%、67.24%、82.76%.平扫与增强的检出率相较有统计学差异(P<0.01);MRI组检出率91.78%明显高于CT组检出率79.45% (P<0.05).结论:MRI检出乙肝肝硬化继发肝癌显著高于CT,临床上二者效结合能够提高乙肝肝硬化继发肝癌诊断率.

Objective:To compare the diagnostic value of MRI and spiral CT in hepatocirrhosis secondary to liver cancer.Methods:A total of 300 patients with hepatitis B cirrhosis admitted to Puyang Hospital of Traditional Chinese Medicine from January 2017 to December 2018 were selected for abdominal spiral CT and MRI examination.To observe the results of liver cancer lesions detected by plain scan and enhanced scan of spiral CT and MRI,and compare the advantages and disadvantages of the two methods in the detection of liver cancer lesions.Results:A total of 67 lesions were detected by MRI,the detection rates of T1 and T2 phases were 70.15%and 73.13%,respectively,and the detection rates of delayed,portal and arterial phases were 74.63%,62.69%and 88.06%,respectively.The difference of detection rate between plain scan and enhanced scan was statistically significant(P<0.05).A total of 58 lesions were detected by CT examination,the detection rate was 53.45%at plain scan stage,70.69%at delayed stage,67.24%at portal vein stage and 82.76%at arterial stage.The difference of detection rate between plain scan and enhanced scan was statistically significant(P<0.01).The detection rate of MRI group was significantly higher than that of CT group(91.78%)(P<0.05).Conclusion:MRI is more sensitive than CT in detecting liver cancer secondary to hepatitis B cirrhosis.
