多西他塞联合雷替曲塞对老年晚期胃癌上皮钙黏素的影响

Effects of docetaxel and raltitrexed on E-cadherin in elderly patients with advanced gastric cancer

目的观察多西他塞与雷替曲塞联合治疗老年晚期胃癌的效果及对上皮钙黏素(E-cadherin)水平的影响。方法选取我院2017年12月至2019年11月收治的90例老年晚期胃癌患者作为研究对象,采用随机数字表法分为2组,观察组45例,行多西他塞与雷替曲塞联合治疗;对照组45例,行多西他塞治疗。2组均治疗2个周期,观察2组患者短期疗效、治疗前后E-cadherin水平及不良反应发生情况。结果治疗2周期后,观察组总有效率(40%)较对照组高(18%),差异具有统计学意义(P<0.05)。2组治疗期间均未发生Ⅲ、Ⅳ级严重不良反应,2组消化系统毒性、血液系统毒性、神经骨骼毒性、心脏毒性等不良反应发生率差异均无统计学意义(P>0.05)。2组患者治疗前E-cadherin水平差异无统计学意义(P>0.05)。治疗2周期后,2组E-cadherin水平均增加,但观察组E-cadherin水平较对照组显著增高,差异均具有统计学意义(P<0.05)。结论多西他塞与雷替曲塞联合治疗老年晚期胃癌效果显著,安全性高,且可增加E-cadherin表达,多西他塞与雷替曲塞联合可用于临床治疗老年晚期胃癌。

Objective To observe the effect of docetaxel and raltitrexed in the treatment of advanced gastric cancer in the elderly and its effect on the level of E-cadherin.Methods Ninety elderly patients with advanced gastric cancer who were treated in our hospital from December 2017 to November 2019 were selected as the research subjects.They were divided into two groups using the random number table method and there were 45 cases in the observation group.The combination therapy was performed in 45 patients in the observation group and 45 patients in the control group were treated with docetaxel.Both groups were treated for 2 cycles.The short-term efficacy,E-cadherin level and adverse reactions before and after treatment were observed in both groups.Results After 2 cycles of treatment,the total effective rate in the observation group(40%)was higher than that in the control group(18%),and the difference was statistically significant(P<0.05).There were no serious adverse reactions of grade Ⅲ and Ⅳ in the two groups during treatment.There was no significant difference in the incidence of adverse reactions such as digestive system toxicity,hematological system toxicity,neuro-skeletal toxicity,and cardiac toxicity between the two groups(P>0.05).There was no significant difference in E-cadherin levels between the two groups of patients before treatment(P>0.05).After 2 cycles of treatment,the E-cadherin level increased in both groups,but the E-cadherin level in the observation group was significantly higher than that in the control group,and the differences were statistically significant(P<0.05).Conclusions The combination of docetaxel and raltitrexed for the treatment of advanced gastric cancer in the elderly has a significant effect,high safety,and can reduce the expression of E-cadherin.The combination of docetaxel and raltitrexed can be used for the clinical treatment of advanced gastric cancer in the elderly.
