前锯肌平面阻滞在乳腺癌手术中的应用效果

Application of serratus anterior plane block in breast cancer surgery

目的:观察对接受手术治疗的乳腺癌患者进行前锯肌平面阻滞的效果。方法:选择在上饶市人民医院接受手术治疗的乳腺癌患者40例作为研究对象。将其随机分为联合组和参考组(20例/组)。对联合组患者进行全身麻醉+前锯肌平面阻滞,对参考组患者进行单纯全身麻醉。然后对比两组患者拔管即刻(T1)、拔管后2 h(T2)、术后24 h(T3)其VAS评分、进行术中、术后镇痛时其芬太尼的使用量、住院的时间和术后慢性疼痛的发生情况。结果:1)在T1、T2和T3,联合组患者的VAS评分均低于参考组患者,P<0.05。2)联合组患者接受术中、术后镇痛时芬太尼的使用量均少于参考组患者,其住院的时间短于参考组患者,其术后慢性疼痛的发生率低于参考组患者,P<0.05。结论:对接受手术治疗的乳腺癌患者进行前锯肌平面阻滞的效果较好,可有效地减少其围手术期芬太尼的使用量,缩短其住院的时间,降低其术后慢性疼痛的发生率。

Objective:To observe the effect of serratus anterior plane block in breast cancer patients undergoing surgical treatment.Methods:40 cases of breast cancer patients who received surgical treatment in Shangrao People's Hospital were selected as study subjects.They were randomly divided into a combination group and a reference group(20 cases/group).Patients in the combined group were given general anesthesia+serratus anterior plane block,and patients in the reference group were given simple general anesthesia.VAS scores,the amount of fentanyl used for intraoperative and postoperative analgesia,the length of hospital stay,and the incidence of postoperative chronic pain were compared between the two groups immediately after extubation(T1),2h after extubation(T2),and 24h after surgery(T3).Results:1)At T1,T2 and T3,VAS scores of patients in the combined group were lower than those in the reference group,P<0.05.2)The dosage of fentanyl in the combined group was less than that in the reference group during intraoperative and postoperative analgesia,the length of hospitalization was shorter than that in the reference group,and the incidence of postoperative chronic pain was lower than that in the reference group(P<0.05).Conclusion:Serratus anterior plane block is effective in reducing perioperative fentanyl use,shortening the length of hospitalization,and reducing the incidence of postoperative chronic pain in patients with breast cancer.
