微创下置管抽吸液化引流术对脑出血患者临床疗效及神经功能的影响

摘要 目的:探讨用微创下置管抽吸液化引流术对脑出血患者进行治疗的效果及对其神经功能的影响。方法:将深圳市宝安区福永人民医院收治的80例脑出血患者随机分为微创手术组(MIS组,n=40)与保守治疗组(CT组,n=40)。对CT组患者进行保守治疗,对MIS组患者进行微创下置管抽吸液化引流术,然后比较两组患者治疗的效果及其各项临床指标。结果:与CT组患者相比,MIS组患者治疗的总有效率更高,治疗后其血清C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)和白细胞介素-6(IL-6)的水平均更低,P<0.05。治疗后14 d,与CT组患者相比,MIS组患者的美国国立卫生研究院卒中量表(NIHSS)评分更低,其Barthel指数评定量表(BI指数)评分更高,P<0.05。结论:用微创下置管抽吸液化引流术对脑出血患者进行治疗能显著减轻其神经功能损伤,改善其血清炎性细胞因子的水平,提高其日常生活活动能力。 Objective:To investigate the effect of minimally invasive subcatheterization on the treatment of cerebral hemorrhage and its effect on the nerve function.Methods:80 patients with cerebral hemorrhage admitted to Fuyong People’s Hospital,Baoan District,Shenzhen were randomly divided into minimally invasive surgery group(MIS group,n=40)and conservative treatment group(CT group,n=40).Patients in the CT group were treated conservatively,while patients in the MIS group were treated with minimally invasive subcatheterization,aspiration and liquefaction drainage.Then the therapeutic effects and clinical indicators of the two groups were compared.Results:Compared with the CT group,the total effective rate of the MIS group was higher,and the serum levels of C-reactive protein(CRP),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)were all lower after treatment,P<0.05.On the 14th day after treatment,compared with the CT group,the MIS group had a lower NIHSS score and a higher Barthel Index Rating Scale(BI index)score(P<0.05).Conclusion:Minimally invasive subcatheterization with suction and drainage can significantly reduce the neurological damage,improve the level of serum inflammatory cytokines and improve the ability of dai ly life in patients with cerebral hemorrhage.
