动态增强磁共振成像定量参数评估乳腺癌新辅助化疗效果及相关性 被引量:12

摘要 目的探讨动态增强磁共振(DCE-MRI)定量参数[转运常数(Ktrans)、细胞外血管外空间的体积分数(Ve)、速率常数(Kep)]评估乳腺癌新辅助化疗(NAC)效果的价值,分析Ktrans、Kep、Ve与肿瘤缩退率的相关性。方法选择189例行NAC治疗的乳腺癌患者,所有患者于治疗前、治疗4 w后行DCE-MRI检查,获得定量参数化疗前Ktrans、Kep、Ve,化疗后Ktrans、Kep、Ve,Ktrans比值、Kep比值、Ve比值。分别以病理完全缓解(pCR)、肿瘤缩退分级(TRG)、肿瘤降期(T降期)为评价NAC疗效的标准,观察不同疗效组上述指标及肿瘤缩退率的差异。Pearson相关分析DCE-MRI定量参数与肿瘤缩退率的相关性,受试者工作特征(ROC)曲线分析DCE-MRI定量参数预测乳腺癌NAC疗效的价值。结果pCR组、TRG缓解组、T降期组患者化疗前Ktrans、Kep、Ktrans比值、Kep比值均大于未pCR组、TRG未缓解组、T未降期组(P<0.05)。化疗前Ktrans、Ktrans比值与肿瘤缩退率呈高度正相关(P<0.05)、化疗前Kep、Kep比值与肿瘤缩退率呈中度正相关(P<0.05),化疗前Ve、Ve比值与肿瘤缩退率无关(P>0.05)。ROC分析结果示Ktrans比值、化疗前Ktrans预测乳腺癌pCR、TRG、T分期的效能高于化疗前Kep、Kep比值,其中预测乳腺癌患者pCR的效能最高,AUC分别达0.983(95%CI:0.967~0.999,P=0.000)、0.936(95%CI:0.892~0.980,P=0.000)。结论DCE-MRI定量参数Ktrans比值、化疗前Ktrans预测乳腺癌NAC治疗后pCR具有较高价值,可以作为乳腺癌患者NAC化疗疗效评价的主要辅助指标。
