一起夹套反应釜封头接管开裂事故原因分析与处理

摘要 使用单位一台在用夹套反应釜,定期检验时在上封头冷却水出口管根部焊缝处发现长度70mm裂纹一条,通过现场查验、着色探伤进行了事故原因分析。分析结果表明:该出口管温度波动频繁,叠加反应釜间歇性操作,所引起的热疲劳导致冷却水出口管根部焊缝出现裂纹。最后给出相应的修理措施和处理,避免以后类似事故再次发生。 During the process of regular inspection, a crack with a length of 70 mm at the weld of the root of the cooling water outlet pipe of the upper head was inspected. The cause of the accident was analyzed through on-site inspection and color inspection. The analysis results show that: the temperature of the outlet pipe fluctuates frequently, and the intermittent operation of the reactor is superimposed. The thermal fatigue damage caused by this causes cracks in the welding seam at the root of the cooling water outlet pipe. Finally, corresponding repair measures and treatments are given to avoid similar accidents in the future.
