社区衰弱老年人轻度认知障碍现状及影响因素分析

Status quo of mild cognitive impairment among the frail elderly in the community and analysis of influencing factors

[目的]了解社区衰弱老年人轻度认知障碍(MCI)现状及其相关影响因素,为后期针对性干预研究提供依据。[方法]采用Fried衰弱表型评估法对社区909例老年人进行衰弱的诊断,对确诊为衰弱的118例老年人采用一般情况调查表、蒙特利尔认知评估量表(MOCA)、日常生活能力量表(ADL)进行MCI初筛,再由神经内科医生进行MCI的临床诊断。[结果]118例社区衰弱老年人中49例为MCI病人,社区衰弱老年人MCI患病率为41.5%。Logistic回归分析发现,年龄、文化程度、智力活动、握力是影响社区衰弱老年人MCI的因素。[结论]社区衰弱老年人MCI现状不容乐观,医护人员应重视社区衰弱老年人MCI的早期筛查,及时采取干预措施,以延缓和减轻衰弱老年人认知功能损伤。

Objective:To investigate the influencing factors and the status quo of mild cognitive impairment(MCI)among the frail elderly in the community,and to provide evidences for targeted intervention research in the future.Methods:Fried frailty phenotype assessment method was used to diagnose frailty in 909 elderly people in the community.For 118 elderly people diagnosed as frailty,general condition questionnaire,Montreal Cognitive Assessment Scale(MOCA)and Activities of Daily Living Scale(ADL)were used for preliminary screening of MCI,and then the clinical diagnosis of MCI was conducted by neurologist.Results:Among 118 older adults with frailty in community,49 were MCI patients,and the prevalence of MCI was 41.5%.Logistic regression analysis showed that age,education level,intellectual activity and grip strength were the important factors affecting MCI of the frail elderly in the community.Conclusions:The status quo of MCI in the frail elderly in the community is not optimistic.Medical staff should pay attention to the early screening of MCI in the frail elderly in the community and take timely intervention measures to delay and reduce the cognitive impairment of the frail elderly.
