丘脑底核脑深部电刺激对改善中晚期帕金森病患者症状及药物用量的短期随访研究 被引量:3

摘要 目的回顾性分析行丘脑底核脑深部电刺激(STN-DBS)手术治疗的15例中晚期帕金森病患者,评估手术疗疗效及安全性。方法收集自2017年1月至2019年1月行双侧STN-DBS治疗的帕金森病(PD)患者资料,比较术前术后运动症状改善情况、认知和精神测试评分变化、每日左旋多巴等效剂量和左旋多巴起效时间的变化。结果术后1年运动症状较术前显著改善,左旋多巴起效时间明显缩短,有效时间延长,左旋多巴日等效剂量明显减少,差异具有统计学意义,术前术后智力精神等无明显变化。结论STN-DBS可以改善中重度帕金森病患者的运动症状和生活质量,对帕金森病的部分非运动症状改善存在不确定性。 Objective To retrospectively analyze 15 patients with Parkinson's disease in middle and late stage who were operated by subthalamic nucleus deep brain stimulation(STN-DBS)and evaluate the efficacy and safety of the operation.Methods The data of PD patients treated with bilateral STN-DBS from January 2017 to January 2019 were collected,and the improvement of motor symptoms,the changes of preoperative and postoperative cognitive and mental test scores,the changes of equivalent dose of L-dopa and the onset time of L-dopa were compared.Results One year after the operation,the motor symptoms were significantly improved,the onset time of levodopa was significantly shortened,the effective time was prolonged,the daily equivalent dose of levodopa was significantly reduced,the difference was statistically significant,and there was no significant change in preoperative and postoperative intelligence and spirit..Conclusion Deep brain stimulation can improve the motor symptoms and quality of life of patients with moderate and severe Parkinson's disease.There was uncertainty on the improvement of some non motor symptoms of Parkinson's disease.
