摘要
目的观察血必净对老年重症肺炎患者血清可溶性髓系细胞触发受体-1(sTREM-1)水平及CPIS评分的影响。方法将31例老年重症肺炎患者随机分为实验组和对照组。对照组予积极查找病原体,并抗感染、清除炎性介质、支持、对症治疗。实验组在对照组基础上加用血必净注射液治疗,7 d为一个疗程。观察治疗前和治疗后第5、7天二组患者血清sTREM-1表达水平及降钙素原(PCT)指标。同时计算临床肺部感染评分(CPIS)。结果治疗后对照组第7天、实验组第5、7天血清sTREM-1表达水平及降钙素原(PCT)指标均下降显著(P<0.05)。实验组较对照组第7天血清sTREM-1表达水平及降钙素原(PCT)指标下降显著(P<0.05)。治疗后第7天2组临床肺部感染评分(CPIS)均下降显著(P<0.05)。实验组较对照组临床肺部感染评分(CPIS)下降显著(P<0.05)。结论对于老年重症肺炎患者,在积极查找病原体并抗感染对症支持治疗基础上早期联合血必净治疗,能够减轻患者的炎症反应,改善肺损伤,降低疾病严重程度。
Objective To observe the effect of Xuebijing on serum soluble myeloid cell trigger receptor-1(sTREM-1)and CPIS score in elderly patients with severe pneumonia.Methods Thirty-one elderly patients with severe pneumonia were randomly divided into experimental group and control group.Control group to actively find the pathogen,and anti-infection,remove inflammatory mediators,support,symptomatic treatment.The experimental group was treated with xuebijing injection on the basis of the control group for 7 days as a course of treatment.Serum strem-1 expression level and procalcitonin(PCT)were extracted from the two groups before and on the 5th and 7th day after treatment.Clinical pulmonary infection score(CPIS)was also calculated.Results After treatment,the serum sTREM-1 expression level and procalcitonin(PCT)indexes of the control group and the experimental group decreased significantly on the 7th day and the 5th and 7th day respectively(P<0.05).Compared with the control group,the serum sTREM-1 expression level and procalcitonin(PCT)index of the experimental group decreased significantly on day 7(P<0.05).On the 7th day after treatment,the clinical pulmonary infection score(CPIS)of both groups decreased significantly(P<0.05).The clinical pulmonary infection score(CPIS)of the experimental group was significantly lower than that of the control group(P<0.05).Conclusion For elderly patients with severe pneumonia,early combination of Xuebijing treatment on the basis of active pathogen search and anti-infection symptomatic support treatment can reduce the inflammatory response of patients,improve lung injury and reduce the severity of the disease.
