膝痹通汤联合热奄包治疗膝关节骨性关节炎的临床疗效观察

Clinical Observation of Xibitong decoction combined with Reyanbao hot bag in the treatment of knee osteoarthritis

目的观察膝痹通汤联合热奄包治疗膝关节骨性关节炎(knee osteoarthritis,KOA)的临床疗效。方法将80例膝关节骨性关节炎患者按照随机数字表分为两组,对照组40例患者采用透明质酸钠膝关节腔注射,观察组40例患者采用膝痹通汤联合热奄包治疗,连续治疗8周后观察西安大略和麦克马斯特大学骨关节炎指数(WOMAC评分),检测膝关节腔液透明质酸(HA)和炎症相关因子(TNF-α、IL-1和IL-6)水平,并进行临床疗效评定。结果治疗后,观察组WOMAC总分和各维度评分较治疗前明显降低(P<0.05),且观察组降低程度大于对照组(P<0.05);观察组膝关节腔液HA水平较治疗前明显升高(P<0.05),且观察组升高程度大于对照组;观察组膝关节腔液TNF-α、IL-1和IL-6水平较治疗前明显降低(P<0.05),且观察组降低程度大于对照组(P<0.05)。观察组总有效率97.50%优于对照组82.50%(P<0.05)。结论采用膝痹通汤联合热奄包治疗膝关节骨性关节炎临床疗效显著,能够明显改善膝关节骨性关节炎症状,防止软骨磨损。

Objective To observe the clinical effect of Xibitong decoction combined with Reyanbao hot bag in the treatment of knee osteoarthritis.Methods 80 patients with knee osteoarthritis were divided into two groups according to the random number table method,40 patients in the control group were injected with sodium hyaluronate in the knee cavity,while 40 patients in the observation group were treated with Xibitong decoction combined with Reyanbao hot bag.WOMAC score was observed after 8 weeks of continuous treatment,and the levels of hyaluronic acid(HA)and inflammation-related factors(TNF-α,IL-1 andIL-6)in the knee joint cavity fluid were detected.The clinical efficacy was evaluated.Results After treatment,the WOMAC total score and each dimension score of the observation group were significantly lower than those before treatment(P<0.05),and the reduction degree of the observation group was greater than that of the control group(P<0.05).The level of HA in the knee joint cavity fluid in the observation group was significantly higher than that before treatment(P<0.05),and the increase degree in the observation group was greater than that in the control group.The levels of TNF-α,IL-1 and IL-6 in the knee joint cavity fluid in the observation group were significantly lower than those before treatment(P<0.05),and the degree of reduction in the observation group was greater than that in the control group(P<0.05).The total effective rate of 97.50%in the observation group was better than 82.50%in the control group(P<0.05).Conclusion The treatment of knee osteoarthritis with Xibitong decoction combined with Reyanbao hot bag has a significant clinical effect,which can significantly improve the symptoms of knee osteoarthritis and prevent cartilage wear.
