输尿管软镜联合钬激光碎石治疗肾结石及输尿管上段结石的临床疗效分析

Clinical efficacy of flexible ureteroscope combined with holmium laser lithotripsy in the treatment of renal calculi and upper ureteral calculi

目的分析输尿管软镜联合钬激光碎石治疗肾结石及输尿管上段结石的临床疗效。方法62例肾结石及输尿管上段结石患者为研究对象,按治疗方式不同将其分为对照组和实验组,各31例。对照组实施经皮肾镜碎石手术,实验组实施输尿管软镜联合钬激光碎石手术。比较两组患者的一次碎石成功率、手术成功率、并发症发生情况、手术时间、住院时间。结果实验组患者的一次碎石成功率和手术成功率分别为93.55%、87.10%,均高于对照组的74.19%、64.52%,差异均具有统计学意义(P<0.05)。实验组患者的并发症发生率为9.68%,低于对照组的32.26%,差异具有统计学意义(P<0.05)。实验组患者的手术时间、住院时间分别为(78.36±10.72)min、(7.41±2.41)d,均短于对照组的(98.26±11.31)min、(12.15±2.85)d,差异均具有统计学意义(P<0.05)。结论治疗肾结石及输尿管上段结石患者,采用输尿管软镜联合钬激光碎石效果显著,能有效提高一次碎石成功率、手术成功率,降低患者并发症发生率,减少手术与住院时间,值得在临床上应用。

Objective To analyze the clinical efficacy of flexible ureteroscope combined with holmium laser lithotripsy in the treatment of renal calculi and upper ureteral calculi.Methods A total of 62 patients with renal calculi and upper ureteral calculi as study subjects were divided into control group and experimental group by different treatment methods,with 31 cases in each group.The control group received percutaneous nephrolithotripsy for treatment,and the experimental group recceived flexible ureteroscope combined with holmium laser lithotripsy for treatment.The success rate of one-time lithotripsy,success rate of surgery,occurrence of complications,surgery time and hospitalization time of the two groups were compared.Results The success rate of one-time lithotripsy and success rate of surgery of the experimental group were 93.55%and 87.10%,which were higher than those of the control group 74.19%and 64.52%,and the difference was statistically significant(P<0.05).The incidence of complications of the experimental group was 9.68%,which was lower than that of the control group 32.26%,and the difference was statistically significant(P<0.05).The surgery time and hospitalization time of the experimental group were(78.36±10.72)min and(7.41±2.41)d respectively,which were shorter than those of the control group(98.26±11.31)min and(12.15±2.85)d,and the difference was statistically significant(P<0.05).Conclusion Flexible ureteroscope combined with holmium laser lithotripsy shows remarkable effect in the treatment of renal calculi and upper ureteral calculi,which can effectively improve success rate of one-time lithotripsy,success rate of surgery,lower the incidence of complications and shorten surgery and hospitalization time.It is worthy of clinical application.
