顺铂联用紫杉醇治疗局部晚期宫颈癌患者的临床效果分析

Clinical effect analysis of cisplatin combined with paclitaxel in the treatment of locally advanced cervical cancer

目的探讨顺铂联用紫杉醇治疗局部晚期宫颈癌患者的临床效果。方法100例局部晚期宫颈癌患者,随机分为对照组与研究组,各50例。对照组予以5-氟尿嘧啶联合顺铂治疗,研究组予以紫杉醇联合顺铂治疗。比较两组患者的治疗效果,病理阳性发生情况,治疗前后瘤体最大直径。结果研究组的治疗有效率80.00%(40/50)明显高于对照组的62.00%(31/50),差异具有统计学意义(P<0.05)。研究组的阴道切缘阳性率4.00%(2/50)、淋巴结转移阳性率8.00%(4/50)、宫旁累及阳性率6.00%(3/50)、脉管浸润阳性率2.00%(1/50)均明显低于对照组的16.00%(8/50)、24.00%(12/50)、20.00%(10/50)、14.00%(7/50),差异均具有统计学意义(P<0.05)。治疗前,两组患者的瘤体最大直径对比,差异无统计学意义(P>0.05);治疗后,两组患者的瘤体最大直径均明显短于本组治疗前,且研究组的瘤体最大直径(3.24±0.31)cm明显短于对照组的(3.67±0.42)cm,差异均具有统计学意义(P<0.05)。结论顺铂联用紫杉醇对于局部晚期宫颈癌患者具有比较满意的临床效果,能明显缩短瘤体最大直径,增强治疗效果并减少病理阳性发生情况,值得临床推广应用。

Objective To discuss the clinical effect of cisplatin combined with paclitaxel in the treatment of locally advanced cervical cancer.Methods A total of 100 patients with locally advanced cervical cancer were randomly divided into control group and research group,with 50 cases in each group.The control group was treated by 5-fluorouracil combined with cisplatin,and the research group was treated by cisplatin combined with paclitaxel.The therapeutic effect,occurrence of pathology positive,maximum diameter of tumor before and after treatment were compared between the two groups.Results The total effective rate of treatment 80.00%(40/50)of the research group was obviously higher than that of the control group 62.00%(31/50),and the difference was statistically significant(P<0.05).The positive rate of vaginal incisal margin,lymph node metastasis,parauterine involvement and vascular invasion of the research group was 4.00%(2/50),8.00%(4/50),6.00%(3/50)and 2.00%(1/50),which was obviously lower than those of the control group 16.00%(8/50),24.00%(12/50),20.00%(10/50)and 14.00%(7/50),and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in maximum diameter of tumor between the two groups(P>0.05).After treatment,the maximum diameter of tumor of the two groups was obviously shorter than those before treatment,and the maximum diameter of tumor of the research group(3.24±0.31)cm was obviously shorter than that of the control group(3.67±0.42)cm,and the difference was statistically significant(P<0.05).Conclusion Cisplatin combined with paclitaxel shows satisfactory effect in the treatment of locally advanced cervical cancer,which can obviously reduce the maximum diameter of the tumor,enhance the treatment effect and reduce the occurrence of pathological positive,which is worthy of clinical promotion and application.
